Abstract
Akutni respiracijski distres sindrom je karakteriziran akutno nastalom hipoksemijom i nekardiogenim plućnim edemom. U djece je ARDS dugo bio zapostavljen entitet pa su tek 2014. definirani dijagnostički kriteriji potrebi za postavljanje dijagnoze. Velika heterogenost i brojnost uzročnika ARDS je dovela do potrebe za podjelom ARDS-a na dva podtipa prema mjestu primarnog oštećenja. U direktnom podtipu ARDS-a primarno oštećenje je na razini alveola, a u indirektnom podtipu na razini endotela krvnih žila. Uzročnici direktnog tipa ARDS-a su pneumonija, virusni bronhiolitis, aspiracija želučanog sadržaja, utapanje, inhalacijska ozljeda. Uzročnici indirektnog podtipa su sepsa, trauma, šok, pankreatitis i akutna ozljeda pluća povezana sa transfuzijom. Prevalencija ARDS-a u djece iznosi 2-12.8 na 100 000 stanovnika godišnje. Djeca svih dobnih skupina mogu oboljeti od PARDS-a. Razlike u prevalencjii nema između spolova. Klinička slika ARDS je obilježena tahipnejom, dispnejom i agitacijom. Pojava dodatnih simptoma ovisi o osnovnoj bolesti koja je dovela do nastanka ARDS. Liječenje pedijatrijskog ARDS-a je usmjereno na liječenje osnovne bolesti koja je dovela do njegovog nastanka, zbog ne postojanja kauzalne terapije ARDS-a što pridonosi visokom mortalitetu. Osnovu terapije čini strojna ventilacija. U slučaju da bolesnik adekvatno ne reagira na terapiju, primjenjuje se izvantjelesna membranska oksigenacija (ECMO). Primjena dodatnih terapijski mogućnosti u vidu inhalacije dušikova oksida i primjene egzogenog surfaktanta također je moguća.
Abstract (english)
Acute respiratory distress syndrome is characterized by acute onset of hypoxemia and noncardiogenic pulmonary edema. Definition and diagnostic criteria were established for children in 2014. High heterogeneity and different etiology of ARDS, were the cause for division in two subtypes, according to the primary damage site. In the direct ARDS subtype, the primary damage is on alveolar level, and in the indirect subtype in vascular endothelial. Causes of direct ARDS are pneumonia, viral bronchiolitis, aspiration of stomach content and drowning. Causes of indirect ARDS are sepsis, trauma, shock, pancreatitis, and acute lung injury associated with transfusion. The prevalence of pediatric ARDS is 2-12.8 cases per 100,000 people per year. Children of all age groups may be affected by PARDS. Clinical picture of ARDS is characterized by tachypnea, dyspnea and agitation. The appearance of additional symptoms depends on the underlying disease leading up to ARDS. The treatment of PARDS is directed towards curing underlying disease, because the lack of causal therapy which contributes to the high mortality of this syndrome. The basic therapy is mechanical ventilation. If the child does not respond adequately, ECMO support could be considered. The use of additional therapeutic possibilities, like nitrogen oxide inhalation and the use of exogenous surfactant is also possible.
