Abstract
Pojam transplantacija označava sve zahvate uzimanja i presađivanja stanica, tkiva i organa. Transplantat može biti vlastiti (autologni), transplantat od donora iste vrste (alogeni), transplantat od strane druge vrste (ksenogeni), ili nastao umjetnim putem što se naziva aloplastični transplantat. Trenutno je “zlatni standard” među transplantatima mekog tkiva autologni slobodni vezivni transplantat. Donorska mjesta za uzimanje autolognih transplantata mekog tkiva u usnoj šupljini su lateralno nepce, zatim tuber maksile te retromolarno područje donje čeljusti. Tehnike koje se danas koriste za preparaciju slobodnih transplantata mekog tkiva su :1) slobodni subepitelni vezivni transplantata uzet tehnikom jednog reza, 2) slobodni subepitelni vezivni transplantata uzet tehnikom “vrata stupice”, 3) slobodni subepitelni transplantat s tubera maksile, 4) slobodni gingivni transplantat deepiteliziran izvan usne šupljine,5) slobodni gingivni transplantat s lateralnog nepca ( “socket seal“ tehnika). Kako bi smanjili postoperacijsku nelagodu i bolnost za pacijenta, koriste se šavovi, udlaga za cijeljenje, PFR, jodoform gazu ili kolagene spužvice. Na taj način pospješujemo cijeljenje mekog tkiva na donorskom mjestu. Iako su danas razvijeni brojni ksenogeni i aloplastični nadomjesni materijali, još uvijek je “zlatni standard” autologni slobodni transplantat vezivnog tkiva
Abstract (english)
The term transplantation refers to all procedures of extracting and transplanting of cells, tissues and organs. A graft can originate from the same person (autologous), from the same species (alogenous), from a different species (xenogenous) or it can be derived artificially and is then called an alloplastic graft. The “gold standard” is the free connective tissue graft. Autologous grafts in oral cavity are mostly harvested from the lateral part of the palate, from the maxillary tuberosity and retromolar portion of the lower jaw. There are several techniques for harvesting free tissue grafts. These are: 1. Free subepithelial connective tissue graft derived from the technique of one cut, 2. Free subepithelial connective tissue graft derived from the technique of the trap door approach, 3. Free subepithelial tissue graft from the maxillary tuberosity, 4. Free gingival graft deepithelialized outside the oral cavity, 5. Free gingival graft from the lateral portion of the palate socket seal technique. In order to decrease postoperative pain and discomfort, stiches, healing appliances, PFR, iodine dressing or different membranes can be used in order topromote healing of the soft tissue after preparation. Despite the development of numerous xenogenous and alloplastic materials, the gold standard is still the autologous free connective tissue graft.
