Abstract
Rak jajnika je maligna bolest koja zahvaća 1/70 žena tijekom života i smatra najsmrtonosnijim zloćudnim tumorom ženskih spolnih organa jer od njega umire 1 % žena. Karcinom ovarija je peti po redu uzrok smrti povezanih sa malignom bolesti u žene. Točan uzrok nastanka raka jajnika ostaje nepoznat u većini slučajeva, no poznati su rizični čimbenici poput rađanja u kasnijoj životnoj dobi, kasnu menopauza, nuliparitet, neplodnost i upotreba lijekova za poboljšanje plodnosti, intrauterina kontracepcija (spirale) te mutacija gena BRCA 1 i BRCA 2. Patohistološki razlikujemo 4 vrste raka jajnika: epitelno – stromalne novotvorine pokrovnog epitela (u koje spada serozni karcinom – generalno najčešći tip karcinoma jajnika), tumori spolnih stanica, novotvorine specijalizirane strome jajnika i metastatski tumori. Tumori graničnog malignog potencijala - „borderline tumori“, se javljaju u mlađih žena i imaju odličnu prognozu. Metode probira na rak jajnika za sada nisu efikasne. Ova bolest daje uglavnom nespecifične simptome. FIGO klasifikacija pomaže u kliničkoj procjeni stadija bolesti. Liječi se operativnim zahvatom uz adjuvantnu kemoterapiju, ako je potrebno. Kod žena oboljelih od karcinoma jajnika, a koje žele zadržati fertilnost, moguć je poštedni operativni zahvat nazvan „fertility sparing surgery“ (FSS). Presudno u odlučivanju o mogućnosti FSS imaju opstetrička i obiteljska anamneza, dok će stadij bolesti biti najbitniji u samoj odluci o poštednom zahvatu. FSS se mora očuvati najmanje uterusa i dio jednog jajnika, a moguće ga je primijeniti samo ako njime ne ugrožavamo uspješnost liječenja. Studije napravljene na temu FSS sugeriraju da je ovaj tip liječenja najsigurniji kod tumora granične malignosti i kod onih koji nisu „clear-cell“ tipa. Kod bolesnica sa stadijem 1A / G3 i 1C clear – cell tumorom se ne razmišlja o poštednom opertivnom zahvatu
Abstract (english)
Ovarian cancer is a malignant disease which affects one out of every 70 women worldwide. It is considered to be the most deadly malignant tumor in the female reproductive system, as 1% of women will die from this disease. It is the fifth cause of death in regards to malignant tumors in women. The main cause of ovarain cancer is unknown, although some risk factors are known, such as givinig birth for the first time at an elderly age, nulliparity, infertility, use of intrauterine contraceptives and the mutation of the BRC1 and BRCA 2 gene. Histologically, ovarian cancer can be divided into 4 categories : epithelial – stromal tumors (the most common type of all ovarian cancer falls into this category - serouse tumor), germ cell tumors, stromal cell tumors and tumors which metastasise the ovaries. Borderline tumors usually affect younger women and have a much better overall survival rate. Screening methods for ovarian cancer are still not effective. Ovarian cancer is presented with non-specific sysmptoms at the start of the disease. FIGO classification is used in clinical assesments of the disease. Surgery is the main method of treatment. Chemotherapy is used when needed. Fertility sparing surgery is used in women who are diagnosed with ovarian cancer who want to preserve their fertility. Obstetric anamnesis and the clinical clasification of the tumor are used whether or not to use this type of surgery. At least one ovary and the uterus must be preserved in FSS, and only if this type of surgery will not interfere with the general outcome of rehabilitation. Studies have shown best results while using FSS for borderline tumors and non clear-cell tumors. Fertility sparing surgery is not used for grade 1A / G1 tumors and grade 1C clear cell tumors
