Epiphyseal cartilage plate are discoid shape, microscopically consists of five zones and are located between the metaphysis and epiphysis of long bones. They are responsible for bone growth in length. In the long bones on both ends are the epiphyseal cartilage plates, while in the smaller bones, such as phalanges only at one end. The bone is formed in two ways: intramembranous ossification and enchondral ossification. Because of enchondral ossification the bone is growing in length. By cessation of growth, the epiphyseal cartilage plates disappear and in their place lags the epiphyseal line. Fractures of epiphyseal growth zones are special forms of fractures and are characteristic for childhood and young adolescent age. There are many classifications of fractures and injuries of epiphyseal growth zones. Salter-Harris classification is the main and describes five types of fractures. Type I and Type V are not classic fractures, type I is the separation of the epiphysis from the metaphysis and type V is a contusion of epiphyseal growth zones. The diagnosis requires anamnestic data, physical examination and x-ray image. Other imaging methods are also used, such as computed tomography, magnetic resonance and ultrasound. Fractures in children and adolescents are most effectively treated conservatively. Operated are only those fractures which must be operated, and then with less metal. The prognosis depends on the severity of the injury, the patient's age, type of injury and the treatment.
