Abstract (croatian) Arterijska hipertenzija najveći je javnozdravstveni problem, kako kod nas, tako i u svijetu.
Europske zemlje, uključujući i Hrvatsku, imaju slične epidemiološke podatke uz značajne
razlike prema SAD-u. Kontrola povišenog krvnog tlaka u naših bolesnika je niska (19,4 %), stoga
je potrebno intenzivirati mjere primarne prevencije i liječenja. Istodobna i kontinuirana
edukacija i bolesnika i liječnika poboljšat će djelotvornost antihipertenzivne terapije, stoga su
stručna društva unatrag nekoliko godina publicirala čitav niz smjernica koje bi nam trebale pomoći
u liječenju bolesnika. Klasifikacija arterijske hipertenzije, kao i potreba za uvođenjem terapije
prema smjernicama Europskog društva za arterijsku hipertenziju i Europskog društva za
kardiologiju iz 2007. godine zasniva se na vrijednostima krvnog tlaka i ukupnom kardiovaskularnom
riziku. Osnova svakog liječenja je pravilna prehrana i promjena životnih navika (pravilno
dozirana svakodnevna umjerena tjelesna aktivnost, redukcija tjelesne težine uz prestanak
pušenja i umjereni unos alkohola u bolesnika koji ga konzumiraju te prehrana koja se bazira na
voću, povrću, bijelom mesu, ribi te višestruko nezasićenim masnim kiselinama i smanjenom
udjelu soli) koja dovodi do pada arterijskog tlaka, a samim time može dovesti i do pada prevalencije
arterijske hipertenzije (i svih njenih komplikacija). Od farmakološke terapije imamo diuretike,
beta blokatore, blokatore kalcijskih kanala, ACE inhibitore i antagoniste angiotenzinskih
(AT1) receptora. U bliskoj budućnosti na našem tržištu trebali bi im se pridružiti aliskiren (direktni
blokator renina) i eplerenon (selektivni blokator aldosteronskih receptora).
Abstract (english) Arterial hypertension is the biggest public health problem in Croatia and abroad.
European countries, including Croatia, have similar epidemiologic data, with significant differences
in comparison to the USA. The level of hypertension control of our patients is very
low (19.4 %), which is why it is necessary to intensify the measures of primary prevention
and treatment. The efficacy of antihypertensive therapy can be improved by simultaneous
and continuous education of both patients and doctors. Professional societies have published
numerous guidelines in the past few years, intended to help us to treat our patients.
Classification of arterial hypertension and the need to introduce therapy according to the
2007 European Society of Hypertension and European Society of Cardiology guidelines is
based on blood pressure values and total cardiovascular risk. Every treatment is based on
correct diet and change in the lifestyle (appropriately dosed moderate daily physical activity,
reduction of body weight, no smoking and moderate consumption of alcohol, as well as a
diet based on fruit, vegetables, white meat, fish and polyunsaturated fatty acids and reduced
intake of salt); this leads to a decrease in the arterial pressure and may consequently
result in reduced incidence of the arterial hypertension (and all the ensuing complications).
Pharmacological therapy includes diuretics, beta blockers, calcium channel blockers, ACE inhibitors
and angiotensin receptor blockers (AT1). In the near future, the offer in our market
should also include aliskiren (direct rennin inhibitor) and eplerenon (selective aldosterone
blocker).
