Abstract
Epileptički status je hitno kliničko stanje u pedijatriji koje se može javiti kod djece bilo koje dobi. Javlja se u omjeru 16-61/100,000 djece, a pojavnost dominantno ovisi o dobi, spolu, socioekonomskim čimbenicima, te osobnim faktorima kao što su urođena stanja i bolesti. Češća je kod djece nego kod odraslih zbog povećane podražljivosti mozga kao posljedice nezrelosti moždanih struktura i sinapsi. Etiologija je šarolika i ovisi dominantno o dobi, a razvoj epileptičkog statusa može biti posljedica urođenih malformacija mozga, traume, infekcije središnjeg živčanog sustava, elektrolitnih disbalansa, ali i visokog febriliteta. Epileptičke statuse dijelimo na konvulzivne i ne-konvulzivne, od kojih se svaki dijeli još na žarišne i generalizirane epileptičke statuse. Za postavljanje konačne dijagnoze, osobito kod ne-konvulzivnih epileptičkih statusa potrebno je učiniti EEG. Terapija epileptičkih statusa uključuje suportivne mjere, liječenje podliježećih uzroka kada oni postoje, te specifično antikonvulzivno liječenje. Antikonvulzivna terapija primjenjuje se u četiri koraka, ovisno o trajanju epileptičkog napadaja, a osnovu liječenja čine benzodiazepini i dugodjelujući antikonvulzivni lijekovi. Produljeni epileptički napadaji i epileptički status mogu imati trajne neželjene posljedice jer uzrokuju smrt i oštećenje neurona, zbog čega je nužno na vrijeme liječiti i prekinuti epileptički status. U rjeđim slučajevima, dolazi i do smrti bolesnika.
Abstract (english)
Status epilepticus is a pediatric emergency that can appear at any age. It appears in 16–61/100,000 children, which mainly depends on a child's age, sex, socioeconomic status, and personal factors, which include congenital malformations and other conditions. Status epilepticus is more common in children than in adults due to excessive hyperexcitation of the brain, resulting from the immaturity of the brain and synapsis. There are different causes of status epilepticus, and they depend mainly on a child's age. It can be a result of congenital malformations, infections, trauma, metabolic disorders, but also high fever. This condition can be divided into convulsive and non-convulsive status epilepticus, and each of them can be further divided into focal and generalized status epilepticus. An EEG result is needed to make a final diagnosis in both. Treatment includes four levels, depending on the duration of the seizure. Benzodiazepines are the basis of treatment, along with long-acting anticonvulsive medication. Status epilepticus can lead to neuronal death and damage and can therefore cause permanent consequences. The probability increases with the duration of the seizure, and therefore medications need to be started as soon as possible. In rare cases, status epilepticus can lead to the patient's death.
