Cesarean section rates are rising in Europe and rest of the world. The most common indications for cesarean section are previous cesarean section, fetal malpresentation, fetal distress and prolonged or obstructed labour. All of them cause constant increase of the number of pregnant women with previous caesarean section. Attempted vaginal birth after caesarean section was introduced in obstetric practice for pregnant women with previous caesarean section, without any additional indications for caesarean section. If vaginal delivery is without contraindications, all pregnant women with prior history of an uncomplicated lower segment cesarean section should be advised and offered the possibility of attempted vaginal delivery. Vaginal birth after a cesarean delivery should be conducted in institutions that can provide timely caesarean section or laparotomy in case of complications during childbirth. Carefull intrapartum care of mother, monitoring progress of labor and continuous electronic fetal monitoring are mandatory. The recommendations for the management of vaginal birth after cesarean section are published in this article. Attempt of vaginal delivery after previous cesarean section is recommended to all pregnant women without new indications for cesarean section and with no contraindications to a vaginal delivery, with carefull monitoring of fetal and mother's status. This should be performed in a safe hospital environment, where all the conditions are fullfiled for emergency actions in case of complications.
