Abstract
Statini, lijekovi za snižavanje kolesterola su nezaobilazni temelj koji se ĉesto propisuju u kliniĉkoj praksi radi prevencije i lijeĉenja kardiovaskularnih bolesti. Iako postoje brojni dokazi o utjecaju statina na snižavanje LDL i ukupnog kolesterola i dugotrajnih uĉinaka kod prevencije kardiovaskularnih bolesti, još uvijek postoje razne predrasude koje su vezane za razne uĉinke lijeĉenja statinima koji su nepoţeljni te njihovu moguću štetnost. Mehanizam djelovanja ovih lijekova je inhibicija hidroksi-metil-glutaril koenzima A reduktaze (HMG CoA), enzima znaĉajnog u procesu sinteze kolesterola. Statini svoje pozitivne uĉinke na smanjenje srĉanožilnog rizika ne ostvaruju samo tako što snizuju razinu kolesterola nego se tu radi i o nizu o kolesterolu neovisnih pleiotropnih uĉinaka. Nuspojave koje se javljaju prilikom lijeĉenja statinima su većinom blage i najĉešće ne zahtijevaju prekid lijeĉenja.
Abstract (english)
Statins are medications for cholesterol reduction who are foundation for prophylaxis and therapy of cardiovascular diseases. Although there are ample evidence on the effect of statins in reduction LDL and total cholesterol and long-term effects in prevention cardiovascular disease, there are still many prejudices related to various effects of statins, which are undesirable and their possible harmful effects. Their main mechanism of action is inhibition of hydroxy-methyl-glutaryl coenzyme A reductase (HMG CoA reductase), an enzyme that is important in the synthesis of cholesterol. Statins positive effects on cardiovascular risk reduction do not achieved only by lowering cholesterol levels but it works on a range of cholesterol independent pleiotropic effects. Side effects that occur during treatment with statins are mild and usually does not require discontinuation of treatment.
