Abstract
Stotinama godina smrt je u većini kultura bila prihvaćena kao čin od boga i kao nešto konačno na što čovjek nije mogao niti imao dopuštenje utjecati. Osnivači modernog koncepta kardiopulmonalne resucitacije koja se sastoji od pritisaka na prsni koš i ventilacije pluća pomoću pozitivnog tlaka čime se na umjetan način održava protok krvi i dotok kisika vitalnim organima su Peter Safar i Jemes Elam. Uspješna reanimacija je moguća samo uz dobro funkcioniranje svih karika u lancu preživljavanja koji... More se sastoji od: rane aktivacije hitne službe, rano započeto osnovnih postupaka održavanja života (BLS), rane defibrilacije, ranih naprednih postupaka održavanja života (ALS) te integrirane poslijeresuscitacijske njege. Osposobljeni spašavatelj bi trebao što brže prići žrtvi te provjeriti njenu reaktivnost i ukoliko žrtva ne reagira i ne diše normalno obavjestiti hitnu službu. Visoko kvalitetna kardiopulmonalna reanimacija i dalje ostaje ključni dio uspješnosti postupka. Smjernice iz 2015. i dalje nalažu dubinu kompresije od 5–6 cm frekvencijom 100–120/min. Dispečer hitne medicine ima važnu ulogu u ranoj dijagnozi srčanog zastoja i provedbu telefonom vođene reanimacije te dostavljanje informacija o najbližoj lokaciji automatskog vanjskog defibrilatora koji bi se trebali nalaziti na što većem broju javnih mjesta. Postupci uznapredovalog održavanja života sadrže KPR, defibrilaciju, prepoznavanje i liječenje potencijalnih reverzibilnih uzroka kardijalnog aresta (4H i 4T), uspostavljanje sigurnog dišnog puta te adekvatnog vaskularnog pristupa, prateći zadani algoritam. Svi građani bi u najboljem slučaju trebali biti u sposobni i uvježbani za standardnu KPR koja uključuje kompresije prsnog koša i ventilaciju u omjeru 30:2. Nove smjernice i preporuke za reanimaciju izdaje ERC svakih pet godina temeljem novih istraživanja i znanstvenih spoznaja. Posljednje smjernice izdane su u listopadu 2015. godine. Less
Abstract (english)
For hundreds of years of death in most cultures was accepted as an act of God as something definite to which man couldn't nor had permission to affect. The founders of the modern concept of cardiopulmonary resucitacije consisting of the chest compression and lung ventilation using positive pressure thereby to artificially maintain blood flow and oxygen supply to vital organs are Peter Safar and Jemes Elam. Successful resuscitation is possible only with the proper functioning of all links in... More the chain of survival, which consists of: early activation of the emergency services, early application of basic life support (BLS), early defibrillation, early advanced life support procedures (ALS) and integrated postresuscitation care. Trained rescuer should as quickly as possible approach the victim and check its reactivity and if the victim is unresponsive, without palpable pulse and not breathing normally inform emergency services. High-quality cardiopulmonary resuscitation remains a key part of the success of the procedure. The guidelines from 2015. still require compression depth 5-6 cm frequency 100-120 / min. Emergency medicine dispatcher plays an important role in the early diagnosis of cardiac arrest and telephone guided resucitation and should provide information on nearby location of automatic external defibrillators, which should be placed on as many public places. Procedures of an advanced life support include CPR, defibrillation, recognition and treatment of possible reversible causes of cardiac arrest (4H and 4T), the establishment of a secure airway and adequate vascular access following a given algorithm. All citizens would at least should be capable and trained in standard CPR that includes chest compression and ventilation with 30:2 ratio. New guidelines and recommendations are issued by the ERC Resuscitation every five years based on new research and scientific knowledge. The last guidelines were issued in October 2015. Less
