Abstract
Ciljevi istraživanja: utvrditi učestalost pojave hiperkloremijske acidoze u kirurških bolesnika u Kliničkom bolničkom centru Rijeka, lokalitet Sušak u 2015. i 2016. godini. Nadalje, ustanoviti povezanost fiziološke otopine i pojave hiperkloremijske acidoze, te posljedičnu pojavnost hiperkloremijske acidoze s obzirom na korištenje nebalansiranih kristaloida u usporedbi s balansiranim kristaloidima, te s obzirom na duljinu trajanja operativnog zahvata i spol pacijenata. Ispitanici i postupci: retrospektivnim istraživanjem obuhvaćeni su svi kirurški pacijenti koji su boravili u Jedinici intenzivnog liječenja u razdoblju od 1. siječnja 2015. do 30. travnja 2016., na sušačkom lokalitetu Kliničkog bolničkog centra Rijeka, od kojih su potom izdvojena 42 visokorizična pacijenta koji su izloženi elektivnom kirurškom zahvatu s boravkom u JIL-u duljim od 24 sata. Rezultati: Učestalost hiperkloremijske acidoze bila je statistički značajno manja u pacijenata iz općeg dijela JIL-a (31,8%) u odnosu na kardiokirurški (75%). Povišenjem volumena intavenski primijenjene fiziološke otopine koncentracija klorida u plazmi raste u statistički značajnoj mjeri, a pH vrijednost plazme pada. Zaključci: u Kliničkom bolničkom centru Rijeka na sušačkom lokalitetu značajna je učestalost hiperkloremijske acidoze u kirurških pacijenata, osobito u pacijenata nakon kardioloških operacija, te se direktno može povezati s velikim volumenima primijenjenih nebalansiranih kristaloidnih otopina.
Abstract (english)
Objective: was to determine the incidence of hyperchloremic acidosis in surgical patients at University Hospital Rijeka, locality Sušak in the years 2015. and 2016.. To establish the connection between the use of saline and the appearance of hyperchloremic acidosis. To determine the difference in the incidence of hyperchloremic acidosis due to the use of unbalanced crystalloid versus balanced crystalloid, and given the duration of the surgical procedure and patient's sex. Subjects and methods: restrospective study included all surgical patients who stayed in the Intensive Care Unit for the period from 1st January 2015. to 30th April 2016., on the site Sušak of Clinical Hospital Centre Rijeka, 42 high-risk patients who underwent elective surgery and stayed in the ICU longer than 24 hours were then isolated and closely looked into. Results: the incidence of hyperchloremic acidosis was significantly lower in patients from the general part of the ICU (31.8%) compared to the cardio-surgical patients (75%). By increasing the intravenous volume of saline, the plasma chloride concentration increases in a statistically significant extent, and the pH value drops. Conclusions: in Clinical Hospital Center Rijeka, Sušak site, there is a significant incidence of hyperchloremic acidosis in surgical patients, particularly in patients after cardiac surgery, and it is directly associated with administering large volumes of unbalanced crystalloid solution.
