Non-small cell lung cancer (NSCLC) accounts for 85% of all lung cancers. Due to the late diagnosis and difficulties in treatment, increasing importance is given to targeted therapy targeting certain receptors, enzymes and components of the immune system within the tumor microenvironment .The microenvironment of solid tumors is complex, consisting of cells that make up stromal and microvascular structures, and those that participate in the immune response.The programmed cell death receptor-1 (PD-1) is a significant co-receptor expressed on T-lymphocytes.Its ligand (PD-L1) can be found expressed on some tumor cells. PD-L1 binding to PD-1 causes inhibition of T-lymphocyte activation and effector functions. Thus, the PD-1 / PD-L1 signaling pathway causes immunosuppression and promotes tumor growth which has been the subject of research by various studies. Macrophages can polarize into M1 and M2 macrophages. Classically activated M1 macrophages play a cytotoxic role, while M2 macrophages show immunosuppressive properties. In our study, we analyzed the expression of the PD-1 molecule on lymphocytes in the stroma of the NSCLC and compared it with different clinicopathological data and survival. Statistical analysis of the data showed a proportional relationship of PD-1 expression with histological grade and pathological stage of the tumor (p=0,050 i p=0,004). Furthermore, PD-1 expression is associated with shorter survival (p=0,029). These results support the role of the PD-1 molecule as a prognostic marker. We analyzed the expression of HLA-DR positive macrophages (M1) and compared it with the same clinicopathological features. The analysis of the results revealed a proportional relationship of HLA-DR expression with proliferative indeks and pathological stage (p=0,007 i p=0,029). The association of higher HLA-DR expression with PD-1 expression has also been demonstrated.These results do not clearly show the role of M1 macrophages in the tumor microenvironment, but suggest their importance
