Burns are injury to the skin and other tissues usually caused by heat. They are significant public health problem causing more than 180 thousand deaths per year. Heat causes denaturation of proteins, activation of the immune system therefore burns occur. Burns depth is classified into four degrees. To asses severity of burns, beside depth, it is necessary to asses affected body surface area. "Rule of nines" and Lund-Browder chart are used for that. Burns are characterized by specific pathophysiological changes that are more pronounced in the more severe clinical presentation (>20-30% TBSA). Changes are divided into two phases – hypodinamic phase and hyperdynamic and hypermetabolic phase. Hypodinamic phase is characterized by depletion of intravascular volume so it is necessary to resuscitate fluids and avoid a fatal outcome. There are many alghorithms for burn resuscitation, but the Parkland formula is the most common one. According to this formula, burn victim should be given four mililiters of Ringer lactate solution per kilogram of body weight per percentage of body surface area burnt. Half of this volume should be given in the first eight hours after burn incident and the rest during following sixteen hours. This formula should not be used exclusively but care of burned patient should be individualised in order to achieve resuscitation endpoints: urine output >0.5 mL/kg/h, base deficit <2 mmol/L, systolic blood pressure 90-120 mmHg, warm extremities, palpable peripheral pulses and the patient should be comfortable.
