Nowadays complete denture is the most common solution in prosthodontic rehabilitation of edentulous patients. Occlusal concepts have to be chosen individually for each patient before teeth setting in complete denture is done. Selection of each occlusal concept depends on: residual alveolar ridge, type of ridge resorption, relationship between the upper and lower jaws, skeletal class of the patient, neuromuscular control, estethic and mastication function and parafunctional movements of the mandible. During the movements, mandible can take centric and eccentric positions. In complete dentures, mandible positions that are necessary to define are centric relation and maximal intercuspidation. Also, morphology of oclussal surface of the teeth can be changed by the factors that are affecting mandibular movements. Generally, those factors are divided into frontal and posterior control factors. Occlusal concepts are defined as specific tooth touches during mandibular movements. There are five types of occlusal concepts: bilateral balanced occlusion, unilateral balanced occlusion, canine guided occlusion, lingualized occlusion and unbalanced occlusion. For each occlusal conception there are certain indications, advantages and disadvantages. Teeth are chosen according to their shape and inclitation of the cusps, after selecting the occlusal concept. We distinguish three types of teeth: anatomical, semi-anatomical and nonanatomical. On occlusal surface of the tooth two types of cusps can be determinated: supporting and non-supporting cusps. Their main role is to detain the vertical and horizontal dimension of occlusion, prevent tissue collision and establish denture stability while mastication. Adequate choice of occlusal concept, tooth and cusp morphology by indication can establish good stability and retention, comfort while wearing complete dentures and satisfying aesthetic and function.
