Temporomandibular disorder includes a wide range of patologic and functional changes in the temporomandibular joint and the surrounding masticatory system. When conservative therapy does not show results, it's an indication for a minimally invasive surgical procedures - arthrocentesis and arthroscopy, which involves intervention in the upper joint compartment. The study included 42 patient hospitalized for surgical treatment of temporomandibular joint disorders at University Hospital Center Rijeka at the Clinic of Maxillofacial Surgery in a five-year period from 2014 to 2018. Most subjects were female, middle-aged and with their own dentition. The left and right sides of TMJ were affected equally. The most common symptoms were pain, sound phenomena and difficulty mouth opening. Previously, they were usually treated with drugs and occlusal splint. A total of 68 minimally invasive surgical procedures were performed. In 78% of cases, arthrocentesis was performed and in the remaining 22% arthroscopy. Surgical procedures have shown a pain reduction of the masticatory muscles in 85%, pain reduction of the head of mandible in 69%, increased mouth opening in 65% and reduction of the sound phenomena in the joint in 54% of the subjects. Postoperative complications were rare, only 9.52%.
