Abstract
Tuberkuloza je najčešća zarazna bolest na svijetu koja odnosi 3 milijuna života godišnje, stara je gotovo koliko i čovječanstvo, te usprskos razvijenoj medicini i znanosti incidencija bolesti je porasla i svakako zaslužuje našu pozornost. Ova bolest zbog načina prijenosa respiratornim putem je česta infektivna bolest i ako se ne liječi može uzrokovati smrt. Rasprostranjena je po čitavom svijetu s najvećim stopama pobola u Africi, Jugoistočnoj Aziji i dijelovima Južne Amerike te u zemljama s lošom epidemiološkom situacijom. Nakon otkrića antituberkulotika diljem svijeta je vladao optimizam te se tuberkuloza smatrala bolešću prošlosti. Međutim, tuberkuloza predstavlja ponovni izazov zbog toga što se javlja „udružena“ s bolestima kod osoba sa narušenim imunitetom kao što su HIV, maligne bolesti, šećerna bolest tj. bolesti moderne civilizacije. Također krajem dvadesetog stoljeća pojavila se i multirezistentna tuberkuloza koja je otporna na glavne antituberkulotike te je produljila hospitalizaciju i dala nove izazove u svijetu medicine i znanosti. Sve je veća pojavnost tuberkuloze u starosti, u vremenu kada se ukupno očekivano trajanje života produžava, te se često prezentira atipičnim simptomima i zahtjeva temeljitu dijagnozu pomoću mikrobioloških metoda. Republika Hrvatska spada u zemlje sa srednjom stopom incidencije tuberkuloze te možemo vidjeti da je u gradu Zagrebu od 2006. do 2015. g. broj oboljelih od tuberkuloze drastično pao zahvaljujući dobroj procijepljenosti te učinkovitom liječenju oboljelih.
Abstract (english)
Tuberculosis is the most common contagious dissease that claims 3 million lives per year. It is almost as old as mankind and, despite modern medicine and the science of incidence, tuberculosis is on the rise and surely demands our attention. Since it spreads via respiratory tract, if not treated in time, this infectious dissease can be lethal. It is present everywhere in the world, but Africa, Southeast Asia, South America and other countries with poorer epidemiological standards demonstrate the highest infection rate. After the discovery of antituberculotics, people were optimistic and tuberculosis was considered as disease of the past. However, tuberculosis nowadays presents a new challenge because it occurs alongside other modern day diseases that compromise the immune system, such as HIV, malignant disseases and diabetes. Furthermore, the late 20th century notes the occurrence of multidrug-resistent tuberculosis, which prolongated hospitalisation and presented the world of science and medicine with new challenges. At a time of increasing life expectancy, the incidence rate of tuberculosis in old age continues to rise and it often manifests itself with atypcial symptoms, therefore demanding a thorough diagnosis through microbiological tests. The Republic of Croatia is one of the countries with medium incidence rate of tuberculosis. The city of Zagreb notes a significant decrease of the affected population, due to high levels of vaccine coverage and effective treatments.
