Abstract
Temporomandibularni poremećaji su poremećaji koji se odražavaju na tkivima žvačnoga sustava: žvačnim mišićima, čeljusnome zglobu, zubima i njihovim potpornim aparatima. Najrašireniji su poremećaji žvačnih mišića zbog kojih se u većini slučajeva pacijenti javljaju svojim liječnicima. Glavni simptom ovih poremećaja je bol. Trijas simptoma koji su karakteristični za TMP-e su bol, zvukovi u zglobu i ograničene kretnje donje čeljusti. U većini slučajeva pacijenti nisu svjesni poremećaja. Uzroci koji dovode do nastanka TMP-a mogu bit lokalnog i sustavnog djelovanja. Lokalni čimbenici su trauma, promjene stanja okluzije, parafunkcijske aktivnosti, a glavni sustavni uzročnik je emocionalni stres. U zbrinjavanju i liječenju TMP-a može se provoditi konačno liječenje kojim se uzroci poremećaja uklanjaju ili kontroliraju, te potporno liječenje kojim se postiže ublažavanje simptoma. Budući da su TMP-i sve više zastupljeni u općoj populaciji, potrebno je upoznati se s potpornim liječenjem koje omogućuje lakše funkcioniranje pojedinog pacijenta s okolinom. U potpornu je terapiju osim farmakoloških metoda uključena i fizikalna terapija. Fizikalna terapija raspolaže manualnim tehnikama i raznim modalitetima u liječenju poremećaja. Manualne tehnike kao što su površinske i dubinske masaže, distrakcija zgloba i mišićno kondicioniranje korisni su u početnom zbrinjavanju poremećaja. Dodatni impuls fizikalnoj terapiji daju modaliteti. Najraširenija je primjena ultrazvuka, a često se koriste termoterapija i terapija hlađenjem. Osim navedenih modaliteta koriste se i ostali: EGS, TENS, laser, iontoforeza, akupunktura kao alternativna metoda. Primjena botulinum toksina postaje sve raširenija, kako u liječenju mišićnih tako i u liječenju zglobnih poremećaja.
Abstract (english)
Temporomandibular disorders (TMD) are disorders of masticatory system and its tissues: masticatory muscles, temporomandibular joint, teeth and their support system. Most common disorders are disorders of masticatory muscles which make patients to visit their doctors of dental medicine. The main symptom of TMD is pain. Triad of symptoms, which are characteristic for TMD, is formed by pain, joint sounds and limited mandibular movement. In most cases patients are not aware of TMD. Causes of TMD can have local or systematic action. Trauma, oclusal changes, parafunction activities are causal factors with local action. The main causal factor with systematic activity is emotional stress. In care and treatment of TMD doctor can use definitive therapy or supportive therapy. Definitive therapy uses methods which removes or controls causal factors. In supportive therapy methods are focused and directed to relieve symptoms. It's important to own a knowledge about supportive therapy because it makes patients symptoms and life easier. Except pharmacal, physical therapy is also a part of supportive therapy. Physical therapy has manual technique and modalities which are useful in treatment of TMD. Surface and deep massage, joint distraction, muscle conditioning are useful manual techniques in initial treatment. Except manual techniques in treatment of TMD can be also used modalities. Modalities such as ultrasound therapy, thermotherapy and criotherapy are most used modalities in treatment. EGS, TENS, cold laser, iontophoresis and acupuncture are also used but not so often as ultrasound and thermotherapy. The use of botulinum toxin is increased in treatment of muscles and joint disorders.
