Endoscopic Retrograde Collangiopancreatography (ERCP) is a technique that combines fluoroscopy and endoscopy for the diagnosis and treatment of pancreatic and gastrointestinal diseases such are stones, strictures and tumors. The most common complication of the procedure is acute pancreatitis that occurs in 2-10% of patients undergoing the procedure. It is thought that the inflammation of the pancreas is the result of hydrostatic lesion and enzyme activation. The result of this activation is the decomposition of the pancreatic tissue and surrounding structures. Pain is the main clinical symptom, localized in the epigastrium and usually spreads to the back accompanied by nausea and vomiting. The patient is anxious and usually hypotensive and tachycardic. Post-ERCP pancratitis is charactarized by a triple increase in serum lipase and amylase levels. The therapy is primarily focused on aggressive hydration and pain reduction. Post-ERCP pancreatitis usually develops a mild form of disease that is self-limiting and resolves through 7 -10 days. In order to prevent the development of pancreatic inflammation after ERCP a number of prophylactic measures, primarily pharmacological, were tested. The drugs that were studied included corticosteroids, somatostatin, NSAID, allopurinol, but also non-pharmacological measures such are stenting of the pancreas canal were also attempted. Positive results of studies studying the influence of indomethacin and diclofenac and the impact of stenting on the pancreatic channel resulted in European Gastroenterology Society distinguishing these two methods as recommendations in their guidelines.
