Abstract
Plućna tromboembolija je važna bolest za sudsku medicinu zbog njezine specifične simptomatologije, te česte prve manifestacije kao iznenadne smrti. Najčešći uzrok nastanka tromba koji progredira u tromboembol je duboka venska tromboza. Za nastanak tromboembola mora biti prisutan jedan ili više uvjeta iz Virchowljevog trijasa: zastoj krvi, oštećenje intime stijenke vene, hiperkoagulabilnost krvi. Dijagnostika plućne embolije je specifična zbog simptoma koji se javljaju kod mnogih drugih bolesti te često dolazi do postavljanja krive dijagnoza. Najvažniji pokazatelj za dokazivanje PE je pozitivan nalaz D-dimera, proteina produkta razgradnje fibrina. Sudskomedicinska obdukcija dokazuje PE kao uzrok iznenadne smrti te traži uzrok nastanka poremećaja zgrušavanja. Sastoji se od makroskopske, mikroskopske i imunohistokemijske evaluacije. Kombinacija novih dijagnostičkih slikovnih metoda poput postmortalne CT ili MR dijagnostike u kombinaciji s histološkim i imunohistokemijskim istraživanjem može integrirati metodološki pristup smrtnim slučajevima u kojima postoji sumnja na PE, kao pomoć da se dođe do ispravne dijagnoze.
Abstract (english)
Pulmonary embolism (PE) is an important disease in forensic medicine due to its specific symptomatology with sudden death often as a first manifestation. The most common cause of thrombus leading to PE is deep vein thrombosis. For thromboembolism to occur, one or more conditions of Virchow’s Triad should be present: blood stasis, endothelial damage and blood hypercoagulability. The diagnosis of PE is specific due to symptoms that also occur in many other diseases, increasing the risk for false diagnosis. The most important marker for PE demonstration is the positive finding of D-dimer, a protein product of fibrin degradation. Immediate autopsy has identified PE as a cause of sudden death and facilitates the investigation of the cause of the clotting disorder, which consists of macroscopic, microscopic and immunohistochemical evaluations. New diagnostic imaging methods, such as postmortem computed tomography (CT) and magnetic resonance imaging (MRI), in combination with histological and immunochemical research can be integrated through a methodological approach for adequate diagnosis in cases of death due to suspected PE.
