Abstract
Mogućnosti lijeĉenja zuba u sluĉaju neuspjele konvencionalne endodontske terapije podrazumijevaju korištenje postupaka endodontske kirurgije. Ti zahvati koriste se u svim situacijama kada se uobiĉajenim endodontskim postupcima ne mogu osigurati uvjeti za cijeljenje patoloških promjena periapikalnih tkiva. Najĉešće se rade postupci apikotomije, a nešto rjeĊe amputacijski postupci (amputacija korijena, hemisekcija, bisekcija). Kada periapikalna patološka promjena zahvaća više od polovice korijena ili je zub parodontološki kompromitiran, jedina mogućnost je njegovo vaĊenje. Postupci endodontske kirurgije u visokom postotku omogućuju uspješno cijeljenje patološki promijenjenog tkiva i oĉuvanje zuba kao funkcijske i estetske jedinice. Korištenjem mikroskopa u suvremenoj dentalnoj medicini i endodontskoj kirurgiji omogućuje se bolja vidljivost operacijskog polja, manje oštećenje tkiva i veće oĉuvanje zdravog tkiva, a samim time povećava se i uspješnost zahvata.
Abstract (english)
If the conventional endodontic treatment fails, the tooth can be treated surgically. Endodontic surgery is performed in all cases when the classic endodontic procedures can not preserve the optimal environment for the periapical lesion's healing. Apicoectomy is the most commonly used procedure, and less often the root amputation, hemisection, and bicuspidization are performed. When the periapical lesion includes more than a half of root length or the tooth is periodontally compromised, the only therapy is tooth extraction. Endodontic surgery allows successful healing of the periapical lesion and maintaining the tooth as a functional and aesthetic unit. Dental microscopes are used in contemporary dental medicine as they provide an optimal visibility of the operating area, the tissue damage is less, and the surgical procedure is performed more successfully.
