Abstract
Period trudnoće i puerperija za trudnicu, odnosno babinjaču predstavljaju period izuzetno povišenog rizika za nastanak venske tromboembolije. Venska tromboembolija tijekom trudnoće i nakon poroda očituje se na dva različita načina, a to su: duboka venska tromboza i plućna tromboembolija. Masivna plućna tromboembolija najteža je posljedica venske tromboembolije tijekom trudnoće i nakon poroda te je najčešći uzrok mortaliteta majki u zemljama kao što su Sjedinjenje Američke Države, Švedska i Velika Britanija. Postoje različiti čimbenici rizika za nastanak venske tromboembolije, a neki od njih su: trombofilija, prethodna venska tromboembolija, neki medicinski komorbiditeti (nefrotski sindrom, diabetes mellitus tip I sa nefropatijom), multipara, varikozne vene itd. U kliničkoj slici plućne tromboembolije prevladavaju slijedeći znaci: tahipneja, povišena tjelesna temperatura, lokalizirani šumovi srca, naglašen drugi srčani ton, tahikardija. Ukoliko je riječ o dubokoj venskoj trombozi u kliničkoj slici nailazi se na slijedeće znakove: bol u nozi koja je zahvaćena dubokom venskom trombozom, pozitivan Hofmanov znak, oteklina noge. Pri dijagnostici duboke venske tromboze metoda izbora je ultrazvuk vena nogu, a pri plućnoj tromboemboliji MSCT angiografija. Liječenje venske tromboembolije tijekom trudnoće i nakon poroda provodi se niskomolekularnim heparinom uzevši u obzir tjelesnu težinu trudnice odnosno babinjače.
Abstract (english)
The period of pregnancy and puerperal period are periods of extremely high risk for venous thromboembolism.Venous thromboembolism during pregnancy and after delivery is manifested in two different ways: deep vein thrombosis and pulmonary thromboembolism. Massive pulmonary thromboembolism is the most severe consequence of venous thromboembolism during pregnancy and after delivery and is the most common cause of mother mortality in countries such as USA, Sweden and the United Kingdom. There are various risk factors for venous thromboembolism, some of which are: thrombophilia, previous venous thromboembolism, some medical comorbidities (nephrotic syndrome, diabetes mellitus type I with nephropathy), varicose veins, etc. The clinical signs of pulmonary thromboembolism are dominated by the following signs: tachypnea, elevated body temperature, accentuated second heart tone, tachycardia. If it is a deep venous thrombosis in a clinical picture, the following signs appear: the pain in the leg which is affected by deep vein thrombosis, a positive Hofman's sign, swelling of the legs. In the diagnosis of deep venous thrombosis, the choice method is ultrasound of the vein, and in pulmonary thromboembolic MSCT angiography. Treatment of venous thromboembolism during pregnancy and after delivery is performed with low molecular weight heparin, taking into account the body weight of the pregnant woman.
