Abstract
Zoonoze su infektivne bolesti zajedničke ljudima i nekim životinjama. Mogu prelaziti sa životinja na ljude te obrnuto. Mnogi su ljudi danas u konstantnoj interakciji sa životinjama, kako u domaćinstvima, tako i van njih. Izvori zoonoza mogu biti kućni ljubimci, domaće životinje ali i životinje s kojima se susrećemo u prirodi. Prema istraživanjima napravljenim od strane Centra za prevenciju i kontrolu bolesti više od 6 od svakih 10 poznatih infektivnih bolesti u ljudi su raširene od životinja, te 3 od svake 4 nove infektivne bolesti u ljudi su raširene također od životinja. Hemoragijska groznica s bubrežnim sindromom (HGSB) je jedna od zoonoza. Uzrokuju je hantavirusi. Način na koji se pojedinac inficira jest inhaliranjem aerosola u kome se nalaze čestice koje sadržavaju virus ili dolaskom u doticaj s kontaminiranim izlučevinama poput mokraće, stolice i sline inficiranih glodavaca. Infekcija zahvaća bubrege i pluća te je to čini ugrožavajućom po život bolesnika. Sam klinički tijek ove bolesti može biti vrlo različit, a neki zaraženi bolesnici mogu proći bez ikakvih simptoma. Simptomi koji se pojavljuju u klasičnom HGSB-u jesu hipotenzije, vrućice, krvarenja, šok te zatajenje bubrega. Manifestiranje simptoma glede bubrežnog zatajenja razlikuje se ovisno o tipu hantavirusne infekcije, a najteže su kod infekcije virusa Hantaan, Dobrava i Seoul, a blaže kod infekcije virusom Puumala koji je najprisutniji u Hrvatskoj. Dijagnoza se vrši na osnovi epidemiološke anamneze o izloženosti glodavcima te kliničke slike pa sve do laboratorijskih nalaza koji upućuju na ovu bolest (trombocitopenija, povišena vrijednost serumskoga kreatinina i ureje, proteineurija, hematurija), a potvrđuje se serološkim i molekularnim dijagnostičkim metodama. Jedino liječenje ove bolesti jest simptomatsko zato što još ne postoji specifična antivirusna terapija za hantavirusne infekcije
Abstract (english)
Zoonozes are infectious diseases common to humans and some animals. They can be transfered from animals to humans and vice versa. Many people today are in constant interaction with animals, both domestically and outside of domestic areas. Sources of zoonoses can be pets, domestic animals, but also the animals we encounter in nature. According to research conducted by the Center for Disease Prevention and Control, more than 6 out of every 10 known infectious diseases in humans are widespread from animals, and 3 out of every four new infectious diseases in humans are also spread by animals . Hemorrhagic fever with kidney syndrome (HGSB) is one of the zoonoses. They are induced by hantavirus. The way an individual infects is by inhalation of an aerosol containing particles containing the virus or coming into contact with contaminated secretions like urine, stool and slime of infected rodents. Infection affects the kidneys and lungs, and this is endangering the life of the patient. The clinical course of this disease can be very different, and some infected patients may pass without any symptoms. Symptoms that occur in classical HGSB are hypotension, fever, bleeding, shock, and kidney failure. Manifestation of renal failure symptoms varies depending on the type of hantavirus infection and is most severe in Hantaan, Dobrava and Seoul virus infections, and milder in infections with the Puumala virus which is most common in Croatia. Diagnosis is made on the basis of epidemiological history of rodent exposure and clinical picture to the laboratory findings suggesting this disease (thrombocytopenia, elevated serum creatinine and urea, proteinuria, hematuria) and confirmed by serological and molecular diagnostic methods. The only treatment of this disease is symptomatic because there is no specific antiviral therapy for hantavirus infections
