Abstract
Sepsa je ozbiljna komplikacija infekcije s vrlo visokim mortalitetom. Sepsa, teška sepsa i septični šok stupnjevi su težine same sepse. Rano prepoznavanje i terapija ključni su za smanjenje smrtnosti. Uvođenjem Protokola u CZHM Rijeka pokušali smo smanjiti vrijeme potrebno za dijagnosticiranje sepse, primjenu adekvatne terapije i pravovremenog liječenja na JIL-u. Ukupno je skupljeno 24 ispitanika od kojih je 11 dijagnosticiranih sepsi. 10 od 11 je premješteno u JIL. Od 11 dijagnosticiranih sepsi 3 su klasificirane kao teška sepsa, 2 kao septički šok, a ostale kao obična sepsa. Protokol je pokazao sposobnost probira sepse i diferenciranja stupnja sepse u ranoj fazi obrade. Nažalost, nismo dobili statistički značajnu razliku u vremenu potrebnom da pacijent sa sepsom dođe u JIL. Naši rezultati se ne poklapaju sa rezultatima ostalih sličnih istraživanja u proteklih nekoliko godina iako pokazuju 53 minute kraće vrijeme do dolaska u JIL. Glavni nedostatak našeg istraživanja je premali broj prikupljenih ispitanika. Predlažemo daljni nastavak provođenja protokola te ponavljanje samog istraživanja nakon jedne ili više godina.
Abstract (english)
Sepsis is serious complication of infection with very high mortality. Sepsis, severe sepsis and septic shock are grades of dividing sepsis by severity. Early recognition and therapy are crucial for mortality reduction. With implementation of the Protocol in CZHM Rijeka we tried to reduce time necessary for diagnosis of sepsis, adequate treatment timing and early transfer to ICU. All together, I have collected 24 evaluated patients from which 11 had diagnosed sepsis. 10 out of 11 were transferred to ICU. From 11 diagnosed sepsis 3 were classified as severe sepsis, 2 as septic shock and others as sepsis. Protocol demonstrated the ability of screening sepsis and differentiation of the sepsis degree in the early stage of processing. Unfortunately, we did not get any statistically significant data in the time needed for patients to come to ICU between experimental and control group. Our results do not match with the results of other similar research in recent years although we did get 53 minutes reduce in time needed for patients to come to ICU . Main deficiency of our research is too small experimental group. Because of that we suggest conducting protocol should be continued for at least a year before repeating this research with prior better informing and motivating medical workers in Emergency Department , University hospital in Rijeka.
