Abstract
Šok je sindrom neodgovarajuće tkivne oksigenacije uslijed smanjene ili neodgovarajuće perfuzije krvi kroz organske sustave, organe i tkiva uz posljedičnu nemogućnost odrţavanja aerobnog metabolizma. Postoje nekoliko podijela šoka. Najbitnija s teapijskoga gledišta je ona s obzitom na kliničku sliku i patofiziologiju. Po toj klasifikaciji postoje četiri vrste šoka: hipovolemijski, kardiogeni, opstruktivni i distribucijski šok. Hemoragični šok je najučestalija vrsta hipovolemijskoga šoka. Njegov... More najčešći uzrok je trauma. Njegov drugi najčešći uzrok je gastroenterološko krvarenje. Velika krvarenja se najčešće pojavljuju u prsnom košu, abdomenu, retroperitonealno i kod velika vanjskih rana. Osnovni znakovi šoka su hipotenzija, tahikardija, oligurija, poremećeno stanje svijesti, hladna, orošena koţa. Osim kliničke slike u dijagnostici su vaţni laboratorijski nalazi i slikovne dijagnostičke metode. Kod obilnih krvarenja ne moţemo se pouzdati u laboratorijske nalaze nego primjenjujemo empirijsku terapiju. Liječenje bolesnika u hemoragičnom šoku uključuje zaustavljanje krvarenja (najčešće kirurškim metodama), nadoknadu intravaskularnog volumena kristaloidnim ili koloidnim otopinama, transfuziju samih pripravka eritrocita i transfuziju pripravka eritrocita zajedno sa plazmom i trombocitima. Masivna tranfuzija krvi predstavlja transfuziju više od 10 jedinica krvi u 24 sata, ili 5 jedinica u 3 sata. Njezinom primjenom mogu se javiti metaboličke komplikacije, koagulopatije i hipotermija. Koagulopatije se javljaju zbog opseţnih oziljeda tkiva i dilucije faktora koagulacije i trombocita s kristaloidnim otopinama ili samim pripravcima eritrocita. Metaboličke komplikacije koje se mogu javiti su alkaloza i hipokalcemija. Zbog transfuzije više od tri jedinice neadekvatno zagrijane krvi moţe doći do hipotermije. Less
Abstract (english)
Shock is a syndrome of inadequate tissue oxygenation due to reduced perfusion of organic sytems, organs and tissues, and consequent unsustainable aerobic metabolism. There are several classification of shock. For therapy, the most important one is the one that includes clinical presentation and pathophyisiology. That classification recognises four classes of shock: hypovolemic, cardiogenic, opstructive and distributive. The most common hypovolemic shock is hemorraghic shock. The most common... More cause of hemorraghic shock is trauma. The second most common cause is gastrointestinal bleeding. Severe bleeding is often found in thoracic cavity, abdomen, retroperitenoal and in large external wounds. The most casual signs of shock are: hypotension, tachycardia, oliguria, abnormal mental status and cold, clammy skin. Beside clinical presentation, in diagnostic of shock, very significant are laboratoy parameters and imaging methods. If bleeding is massive and ongoing, laboratory measurements can be inaccurate, then empiric therapy should take place. Hemmoraghic shock treatment includes stopping bleeding (most often with surgery), Intravenous fluid resuscitation with crystalloids or colloids, transfusion of red blood cells, transfusion of red blood cells with clotting factors and platelets. Massive transfusion is defined as the replacement by transfusion of 10 units of red cells in 24 hours, or five units over three hours. With massive transfusion, complications such as coagulopathy, hipotermia and metabolic complications can occur. Coagulopathies can occur with massive tissue damage and with diluton of clotting factors and platelets with crystaloids and colloids. Metabolic complications are metabolic alkalosis and hypocalcemia.
