Abstract
Polipi su dobroćudne lezije s niskim potencijalom maligne transformacije. Tijekom reproduktivnog razdoblja, budu dijagnosticirani u simptomatskih pacijentica s abnormalnim uterinim krvarenjem ili neplodnosti. Tijekom postmenopauzalnog razdoblja, većina njih su asimptomatski i povezani s abnormalnim krvarenjem u 30% slučajeva. Oni su češći nakon menopauze te se čimbenici rizika za karcinom endometrija ne smatraju faktorima rizika za endometrijske polipe, iako su polipi hormonski ovisni, kao što je slučaj kod pacijentica koje uzimaju tamoksifen. Endometrijski polipi mogu biti dijagnosticirani ultrazvukom, infuzijskom sonografijom ili histeroskopijom, što je zlatni standard. Infuzijska sonografija (SIS), koja koristi normalnu sterilnu fiziološku otopinu za distenziju endometrijske šupljine i pregled polipa je vrlo osjetljiva metoda koja se dobro podnosi, brza je i sigurna te minimalno invazivna dijagnostička tehnika. Njihova definitivna dijagnoza se donosi histološkim pregledom uzorka koji se najučinkovitije dobiva histeroskopski usmjerenom biopsijom, a njihovo najučinkovitije liječenje je postupak histeroskopske resekcije. Polipi se mogu ponovno pojaviti nakon liječenja. Polipektomija je vrlo zadovoljavajući postupak nakon menopauze, ali ima nižu stopu uspjeha kod simptomatskih žena reproduktivne dobi, te poboljšava stopu fertiliteta kod neplodnih žena.
Abstract (english)
Polyps are benign lesions with a low potential for malignant transformation. During the reproductive period, they are diagnosed in symptomatic patients with abnormal uterine bleeding or infertility. During the postmenopausal period, most of them are asymptomatic and are associated with abnormal bleeding in 30% of cases. They are more frequent after menopause and the risk factors for cancer of the endometrium are not considered to be risk factors for endometrial polyps, although polyps are hormone dependent, as is the case among patients taking tamoxifen. Endometrial polyps can be diagnosed with ultrasound, saline infusion sonogram or hysteroscopy, which is the gold standard. Saline infusion sonogram (SIS), which uses a sterile normal saline solution to distend the endometrial cavity and outline polyps, is a highly sensitive, well-tolerated, safe, rapid and minimally invasive diagnostic technique. Their definitive diagnosis is made by the histologic examination of a sample that is most efficiently obtained by a hysteroscopically directed biopsy, and their most effective treatment is by a hysteroscopic resection procedure. Polyps may recur after treatment. The polypectomy is a highly satisfactory procedure after menopause, but has a lower success rate among symptomatic women of reproductive age, and improves the fertility rates of infertile women.
