Abstract
U posljednjih dva do tri desetljeća zamijetio se znatan porast ljudi u svijetu koji su pogođeni alergijama. Naime u svijetu 30 - 40% osoba pati od nekog oblika alergije. Zašto je došlo do tog porasta ne može se još sa sigurnošću objasniti. Današnji znanstvenici pokušavaju tu pojavu objasniti higijenskom hipotezom. Alergene mogu predstavljati bezbroj tvari: određene namirnice, kućna prašina, biljke, ubodi insekata, dlake životinja, kozmetički preparati, lijekovi, metali, dentalno medicinski materijali, puno drugih materijala i stvari kojima smo okruženi u slobodno vrijeme ili u profesionalnom životu. Razlikuju se četiri oblika reakcija preosjetljivosti. Tip I ili rana preosjetljivost posredovana IgE protutijelima, tip II takozvana citotoksična reakcija koja je rezultat vezanja antigena na protutijelo te formiranja kompleksa antigen - protutijelo i aktivacije citotiksičnih t- limfocita. Tip III je posljedica vezivanja IgG i IgM protutijela na antigene i formiranje cirkulirajućih imunokompleksa, koji dovode do oštećenja tkiva i posljedna reakcija preosjetljivosti odnosno tip IV, reakcija posredovana T- limfocitima. Svaka od tih reakcija ima određene manifestacije na oralnoj sluznici, prema kojima ih možemo razlikovati i liječiti odgovarajućim lijekovima. Pri tome moramo posebice poznavati stanja koja su životno ugrožavajuća kao anafilaktički šok, SJS i TEN. U slučaju takvih reakcija organizma na postojeće antigene potreban je brzi početak liječenja i u budućnosti izbjegavanje sredstava tzv. triggera koji izazivaju alergijsku reakciju. Ostala stanja koja spadaju pod alergijske reakcije također zahtjevaju detekciju triggera i lokalno i sustavno liječenje do povlačenja simptoma.

In the last two or three decades it has been noticed that an increasing number of people suffer from allergies. 30-40% of people in the world suffer from one kind of allergy. The reason for this phenomenon is unkonw. Todays scientists try to explain this with the hygiene hypothesis. Certain groceries, dust, plants, insect sting, animal`s hair, cosmetic products, drugs, metal, dental materials and a lot of other things can represent alergenes in the daily and profesional life. One distinguishes between four types of allergic reactions. Typ I or immediate hipersinsitivity caused by antibody IgE, typ II so called cytotoxic hipersentitivity is the result of an antigene binding to an antibody and forming an antigene- antibody complex, thus resulting in an activation of cytotoxic T- lymphocytes. Typ III is the consequence of IgG and IgM antibodies binding to antigenes and forming circulating immune complexes which cause tissue damage and the last reaction, typ IV is an allergic recation caused by T- lymphocytes. Every of these recations have particular manifestations on the oral mucosa, which they can be distinguished by and treated with an accurate therapy. Therefore it is important to know the conditions which can lead to death like anaphylactic shock, Steven- Johnson syndrome (SJS) and toxic epidermal necrolysis (TEN). In case of such reactions of the organism to existing antigenes, it is most important to immediatly start treatment and avoid recsources, so calles triggers, which lead to allergic reactions. Other conditions that are categorized as allergic reactions require too detection of triggers and treatment with local and systemic therapy until symptoms are cured.
