Abstract
Uvod: Ozljeda mozga je vrlo česta ozljeda s brojnim simptomima i posljedicama, a nastaje kao rezultat vanjske mehaničke sile. Broj bolesnika je puno veći od očekivanog. Svrha rada: Prikazati mehanizam ozljeđivanja, liječenje i ishod liječenja bolesnika s teškom ozljedom mozga liječenih u JIL-u Klinike za anesteziologiju i intenzivno liječenje, KBC Rijeka u razdoblju od 1. siječnja 2013. godine do 31. prosinca 2014. godine. Ispitanici i metode: U ovo istraživanje uključeno je ukupno 100 bolesnika s teškom ozljedom mozga svrstanih u dvije skupine prema razdoblju liječenja u JIL-u. Retrospektivnom analizom obrađeni su opći i specifični podaci iz medicinske dokumentacije bolesnika. Glavni kriterij selekcije bio je GCS <8, odnosno dijagnoza teške ozljede mozga. Rezultati: Statistički značajno je bilo više bolesnika s teškom ozljedom mozga muškoga spola i u dobnoj skupini od 19-64 godine života. Najveći broj bolesnika zadobio je tešku ozljedu mozga prilikom padova. Velikom broju bolesnika s teškom ozljedom mozga postavljen je sustav za mjerenje ICP-a. Skupina bolesnika s teškom ozljedom mozga liječena tijekom 2014. statistički je duže boravila u JIL-u, duže je strojno ventilirana i analgosedirana u odnosu na skupinu bolesnika s teškom ozljedom mozga liječenih tijekom 2013 godine. Statistički je značajno više bolesnika s teškom ozljedom mozga umrlo tijekom 2013. godine u odnosu na 2014. godinu. Zaključak: Mehanizam ozljeđivanja ima veliki utjecaj na liječenje i ishod bolesnika s teškom ozljedom mozga. Rano prepoznavanje i liječenje bolesnika s teškom ozljedom mozga poboljšava njihovo preživljavanje.

Introduction: Brain injury is a very common injury with a number of symptoms and consequences. The number of patients is much higher than expected. Objective: The main objective of this study was to present the mechanism of injury, treatment and outcome of patients with severe brain injury treated in the intensive care unit of the Department of Anesthesiology and Intensive Care Medicine, Clinical Hospital Center Rijeka between January 1st 2013 and December 31st 2014. Participants and methods: This study included 100 patients, which were divided into two groups based on the treatment period in the ICU. A retrospective analysis of general and specific data from medical records was performed. The main criteria for selection was GCS <8 and diagnosis of severe brain injury. Results: Significantly, there were more male patients with severe brain injury at the age of 19-64 years. The main mechanism of severe brain injury were falls. The majority of the patients with severe brain injury got an ICP device. Patients with severe brain injury in group 2014 had statistically significant longer length of stay in ICU, were longer mechanically ventilated and analgosedated when compared to group 2013. Statistically significant more patients with severe brain injury in group 2013 died than in group 2014. Conclusion: The mechanism of injury has an important impact on the treatment and outcome of patients with severe brain injury. Early recognition and treatment of patients with severe brain injury improves their survival.
