Abstract Cilj istraživanja: Usporediti gustoću i morfologiju endotelnih stanica rožnice te središnju
debljinu rožnice između pacijenata sa šećernom bolesti tipa 2 koji puše te nepušača, sa
zdravim pojedincima u dobi od 50 do 70 godina, te utvrditi da li pušenje povećava
negativan učinak šećerne bolesti tipa 2 na navedene parametre rožnice.
Ispitanici i metode: Ova prospektivna i kohortna studija je obuhvatila 200 pacijenata
podijeljenih u četiri skupine: pacijenti sa šećernom bolesti tipa 2 koji puše (skupina 1),
nepušači sa šećernom bolesti tipa 2 (skupina 2), zdravi pojedinci koji puše (skupina 3) te
zdravi ispitanici koji ne puše (kontrolna skupina, skupina 4). Za mjerenje gustoće
endotelnih stanica rožnice (eng. endothelial cell density ECD), koeficijenta varijacije
stanične površine (eng. coefficient of variation of cell area CV), postotka šesterokutnih
stanica (eng. percentage of hexagonal cells HEX) i središnje debljine rožnice (eng. central
corneal pachymetry CCT) korišten je bezkontaktni spekularni mikroskop.
Rezultati: Prema vrijednostima ECD -a i CCT -a (P <0,001, odnosno P = 0,013), uočena
je značajna razlika između skupina. Prosječni ECD bio je najniži u pušača sa dijabetesom
(1917 ± 399 stanica / mm2
). Zdravi pušači i pušači sa dijabetesom su imali značajno niži
ECD u usporedbi s kontrolnom skupinom (P = 0,03 i P <0,001, respektivno). Zdravi
pušači i pušači sa dijabetesom su imali značajno niži ECD u usporedbi sa nepušačima sa
dijabetesom (P = 0,012 i P <0,001, respektivno). Rožnica je bila značajno deblja u pušača
sa dijabetesom nego u kontrolnoj skupini (P = 0,013).
Zaključak: Zajedničko međudjelovanje šećerne bolesti tipa 2 i pušenja uzrokuje značajno
smanjenje ECD -a i povećanje CCT -a. Pušenje cigareta izaziva štetniji učinak na
endotelne stanice rožnice u odnosu na šećernu bolest tipa 2.
Abstract (english) Objectives: To compare the corneal endothelial morphology and cell density of diabetic
smokers and non-smokers with 50–70 age-matched healthy subjects and to determine
whether smoking increases the effects of type 2 diabetes mellitus (DM) on these corneal
parameters.
Patients and Methods: This prospective cohort study included 200 patients who were
assigned to four groups, including smokers with type 2 DM (group 1), nonsmokers with
type 2 DM (group 2), healthy smokers (group 3), and healthy nonsmokers (control group,
group 4). Noncontact specular microscopy was used to measure central endothelial cell
density (ECD), coefficient of variation of cell area (CV), percentage of hexagonal cells
(HEX), and central corneal pachymetry (CCT).
Results: According to the ECD and CCT values (P < 0.001 and P = 0.013, respectively),
a significant difference was observed between the groups. The mean ECD was lowest in
diabetic smokers (1917 ± 399 cells/mm2
). Healthy smokers and diabetic smokers had
significantly lower ECD compared to the control group (P = 0.03 and P < 0.001,
respectively). Healthy smokers and diabetic smokers had significantly lower ECD
compared to diabetic nonsmokers (P = 0.012 and P < 0.001, respectively). The cornea was
significantly thicker in the diabetic smokers than in the control group (P = 0.013).
Conclusion: The coexistence of DM and smoking causes a significant decrease in ECD
and an increase in CCT. Cigarette smoking is more harmful to corneal endothelial cells
than DM alone.
