Abstract
Opekline su ozljede kože ili drugih organa uzrokovane toplinom, radijacijom, električnom strujom, trenjem ili kontaktom s kemikalijama. Vrlo su česte u djece, a najčešće se radi o opeklinama zadobivenim kontaktom kože s vrućom tekućinom. Opekline se po dubini dijele na 4 stupnja. Opekline I° zahvaćaju samo epidermis i ne zahtijevaju liječenje. Opekline IIA° zahvaćaju epidermis i površni dio dermisa, a njihovo liječenje obično se sastoji u nježnom debridmanu, sprječavanju infekcije i osiguravanju vlažnog okoliša koji pogoduje reepitelizaciji. Opekline IIB° zahvaćaju epidermis i duboki dio dermisa. Liječenje se obično započinje konzervativnim metodama, a nastavlja kirurški u slučaju odgođenog cijeljenja. Opekline III° zahvaćaju punu debljinu kože. Osnovna metoda u njihovom liječenju je tangencijalna ekscizija do u zdravo s nadomještanjem defekta kožnim autotransplantatom djelomične ili pune debljine kože. Ova osnovna kirurška metoda može biti nadopunjena brojnim suvremenim načinima liječenja kao što su privremeni biosintetski dermalni nadomjesci, uzgojeni epitelni autotransplantat i autologna suspenzija kožnih stanica. Opekline IV° zahvaćaju duboka tkiva kao što su mišići i kosti, a liječenje je iznimno složeno i ovisi o zahvaćenim strukturama. Cirkumferentne opekline, inhalacijska ozljeda i infekcije opeklinske rane česti su u opečene djece, a zahtijevaju promptno liječenje jer bitno pridonose morbiditetu i mortalitetu. U liječenju djece s opeklinskom ozljedom posebnu pažnju valja posvetiti opeklinskoj bolesti, skupu patoloških sustavnih manifestacija opeklinske ozljede. Opeklinska je bolest patofiziološki kompleksna i zahvaća sve organske sustave, a njene su osnovne značajke hipermetabolizam, hipovolemija i velik rizik od nastanka sepse. Nadoknada volumena, nutritivna potpora i kontrola infekcija ključ su liječenja bolesnika s opeklinskom bolešću.
Abstract (english)
A burn is an injury to the skin or other organic tissue caused by heat, radiation, electricity, friction or contact with chemicals. Burns are common in children, with scalds caused by hot liquids being the most common form. Burns are traditionally divided into 4 degrees depending on the depth of skin involved. I° burns affect only the epidermis and do not require treatment. IIA° burns affect the epidermis and the superficial part of the dermis. They are usually treated by gentle debridement followed by providing a moist environment which enhances reepithelization. IIB° burns affect the epidermis and the deep layer of dermis. They can be treated by conservative methods but are usually treated surgically. III° burns affect full thickness of the skin. Tangential excision and grafting are usually implemented in treating patients with III° burns. IV° burns affect deep underlying tissues like muscle or bone. Their treatment depends on particular tissues involved. Circumferential burns, inhalation injury and infections of the burn wound are common in burnt children and require prompt treatment because of their significant contribution to morbidity and mortality. When treating children with severe burns, spotting early signs of pathological systemic manifestations is crucial. These manifestations are pathophysiologically complex and affect all organ systems, with hypermetabolism, hypovolemia and susceptibility to sepsis being particularly important. Volume resuscitation, nutritional support and infection control are key in treating patients with pathological systemic manifestations to a severe burn.
