Abstract
UVOD: Neuroendokrini tumori gastrointestinalnog sustava jesu novotvorine podrijetla neuroendokrinih stanica. Mogu proizvoditi hormone i prouzročiti karcinoidni sindrom. Počinju kao hiperplazija u organu iz koje mogu nastati neuroendokrini tumori ili karcinomi. Vedina ih eksprimira somatostatinske receptore na membranama stanica (SSTR-2A). Klasifikacija WHO iz 2010. godine dijeli neuroendokrine tumore određujudi gradus po broju mitoza i proliferacijski indeks. CILJ: analiza 86 slučajeva neuroendokrinih tumora. MATERIJALI I METODE: bioptički uzorci su pregledavani nativno obojeni i obojeni imunohistokemijskim metodama pomodu raznih komercijalnih protutijela. REZULTATI: vedina bolesnika su muškarci (59%) srednja dobi oko 61 godine. NET-ovi crvuljka javljali su se nešto ranije, sa 43 godine. Najčešde sijelo tumora su bili želudac (22%) i debelo crijevo (20%). Prema WHO klasifikaciji 33 su spadala u kategoriju NET G1, 26 u NET G2, 25 u NEC i 2 u MANEC. Čak 50 tumora je eksprimiralo somatostatinske receptore na membranama stanica, 10 samo u citoplazmi, a 26 ih nije eksprimiralo. Tumori slijepog crijeva i crvuljka su najjače eksprimirali somatostatinske receptore, a želudac, debelo i tanko crijevo najslabije. Analiza odnosa gradusa tumora i ekspresije SSTR 2A pokazala je da postoji negativna korelacija tj. tumori nižeg gradusa imaju jaču ekspresiju SSTR 2A i obrnuto. ZAKLJUČAK: iz analize se može zaključiti da se ovi tumori mogu očekivati kod starijih muškaraca, ranije u crvuljku. Analozi somatostatinskih receptora su adekvatna terapija za ove novotvorine, pogotovo u slijepom crijevu i crvuljku. Incidencija ovih novotvorina je u porastu pa se može očekivati rast broja dijagnoza.

INTRODUCTION: Neuroendocrine tumors of the gastrointestinal tract are neoplasms that arise from the neuroendocrine cells. They can produce hormons and cause carcinoid syndrome. They start as a hyperplasia in an organ from which more agressive neoplasms can arise. Most of them express somatostatin receptors on the membranes of their cells (SSTR-2A). OBJECTIVE: to analize 86 cases of neuroendocrine tumors. MATERIALS AND METHODS: biopsies that were analyzed were routinely colored and colored with various immunohistochemichal methods. RESULTS: most of the patients were male (59%) with a mean age at approximately 61 years. NETs of the appendix occured earlier, at around 43 years. The most common localization was the stomach (22%) and the colon (20%). According to the WHO classification 33 tumors were cthegorized as NET G1, 26 as NET G2, 25 as NEC and 2 as MANEC. Fifty tumors expressed somatostatin receptors on the cell membrane, 10 only in the cytoplasm and 26 didn't express them. Appendiceal and caecal tumors expressed them significantly, while tumors from stomach, small and large intestine expressed them poorly. Gradus – somatostatin receptor 2A analysis showed that tumors with lower grading expresses SSTR 2A more strongly and vice versa. CONCLUSION: from this analysis it can be concluded that these neoplasms can be expected in elder men, earlier in the appendix. Somatostatin receptor analogues are the adequate therapy for these tumors, especially in the caecum and appendix. Incidence of these neoplasms is on the rise so there is an estimated increase in the number of diagnoses.
