Abstract
Opekline su vrsta ozljede koţe i/ili potkoţnog tkiva koje mogu biti uzrokovane toplinom, električnom strujom, kemijskim tvarima, trenjem ili radijacijom. One predstavljaju globalni javnozdravstveni problem. Učestalost opeklina varira u različitim zemljama – najveća je u zemljama u razvoju dok u razvijenim zemljama nisu toliko česte, ali kada se dogode nerijetko uzrokuju doţivotne posljedice za zdravlje unesreće osobe te predstavljaju veliki teret zdravstvenog sustava bilo koje drţave zbog dugotrajne hospitalizacije i rehabilitacije. U patofiziologiji opeklina najvaţniju ulogu ima izraziti gubitak intravaskularnog volumena i proteina što moţe imati za posljedicu razvoj šoka, zatajenje brojnih organa (bubrezi, jetra) te smrt. Primarno liječenje je usmjereno upravo na prevenciju nastanka ovih posljedica. Opekline se dijele na stupnjeve s obzirom na dubinu patohistoloških promjena na koţi te se na temelju toga odreĎuje način liječenja. Manje i lakše opekline se samo ispiru, očiste, premaţu antibiotskom masti i previju, dok se velike i teške opekline zbrinjavaju kirurški. Kirurško liječenje opekline započinje ekscizijom opečenog dijela koţe na koji se onda postavlja slobodni koţni transplantat, najčešće tipa Thierscher u mreţastom obliku. Osim takvog koţnog transplantata opečeno područje se privremeno moţe prekriti i sintetskim proizvodima. Novija metoda u liječenju opeklina jest in vitro uzgoj ljudske koţe i njena pohrana u banke tkiva što upravo i predstavlja budućnost u zbrinjavanju opeklinskih ozljeda.

Burns are a type of injury to the skin and/or subcutaneous tissue that may be caused by heat, electricity, chemical, friction or radiation. They represent a global public health problem. The frequency of burns varies in different countries - the largest is in developing countries while in the developed ones they are not as frequent, but when they happen they often cause lifelong health consequences in an affected person and they represent a burden on the health system of any country due to prolonged hospitalization and rehabilitation. In the pathophysiology of burns most important role has the loss of intravascular volume and proteins which may result in the development of shock, failure of various organs (kidney, liver) and death. The primary treatment is directed precisely on the prevention of these consequences. Burns are divided into degrees based on the depth of the histopathological changes in the skin and on that basis treatment is determined. Smaller and less severe burns need to be washed, cleaned, treated with antibiotic ointment and banded, while the large and severe burns have to be managed surgically. Surgical treatment of burns begins with excision of burned part of the skin which is then covered with free skin graft, the most common type is Thierscher graft in mesh form. In addition to such skin grafts, burned area may be temporarily covered with synthetic products. Newer methods in the treatment of burns is the in vitro cultivation of human skin and its storage in the tissue bank what is exactly the future in the care of burn injuries.
