Abstract
Cilj: Ciljevi ovog istraživanja bile su usporediti status mEGFR-a sa veličinom tumora glotisa prema TNM klasifikaciji, istražiti imaju li pacijenti kod kojih su prisutne metastaze više ili manje izražen mEGFR, te odrediti da li veće vrijednosti mEGFR-a povećavaju potrebu za disekcijom vrata i zračenjem, ako i da, utvrditi izvode li se češće totalne zahvate, laringektomije kod viših vrijednosti mEGFR-a. Ispitanici i metode: Ovo je retrospektivno istraživanje u koje je bilo uključeno 20 ispitanika u dobi od 53 do 81 godina koji su se liječili zbog planocelularnog karcinoma glotisa u KBC-u Rijeka. Ekspresija mEGFR-a u tumorskim stanicama određivala se pomoću imunohistokemije. Rezultati: Rezultati su pokazali da je veći TNM povezan s većim vrijednostima mEGFR-a. Usporedba vrijednosti mEGFR-a prema skupinama tumora, pokazala je da je vrijednost mEGFR-a bila statistički značajno viša kod kasnih karcinoma. Vrijednosti mEGFR-a kod ispitanika sa metastazama bile su više od vrijednosti kod ispitanika bez metastaza, no razlika nije bila statistički značajna. Razina m-EGFR-a nije bila povezana sa odlučivanjem o potrebi za disekcijom vrata. S obzirom na vrstu operacije, pokazalo se da je u skupini s učinjenom totalnom laringektomijom razina mEGFR-a bila je statistički značajno viša. Razina mEGFR-a nije bila povezana s odlučivanjem o potrebi primjene zračenja. Zaključak: Ekspresija mEGFR-a povezana je sa lošijom prognozom bolesti te su potrebna daljnja istraživanja kako bi se poboljšale mogućnosti terapije, preživljenje te kvaliteta života bolesnika.

Aim: The aims of this study were to compare mEGFR status with glottic tumor TNM classification, to determine whether patients who present metastases have more or less expressed mEGFR, and to determine whether higher expression of mEGFR increases the need for neck dissection and radiation, if yes, to determine whether total laryngectomy is more often performed due to increased mEGFR. Patients and Methods: This is a retrospective study which included 20 patients aged 53-81 years who were treated for glottic squamous cell carcinoma at the Clinical Hosptal Center Rijeka. mEGFR expression level in tumor cells was measured by immunohistochemistry. Results: The results showed that the higher TNM was associated with higher expression of mEGFR. Comparison of mEGFR expression with tumor groups showed that mEGFR expression was significantly higher in advanced glottic cancer. mEGFR expression in patients with metastases was higher than mEGFR expression in patients without metastases, but the difference was not statistically significant (due to the small sample size further research is needed). m-EGFR expression was not associated with the decision if neck dissection is needed. Considering the type of operation, it was obvious that in the group with undergone total laryngectomy mEGFR expression was significantly higher. mEGFR expression was not associated with the decision if radiation is needed. Conclusion: The expression of mEGFR is associated with a worse prognosis and further research is needed in order to improve the abilities of treatment, survival and quality of life in patients with glottic cancer.
