Abstract
Rak debelog crijeva po pojavnosti je jedan od najučestalijih zloćudnih oboljenja i najčešći uzrok smrti među malignim bolestima u svijetu. Incidencija raka debelog crijeva u Hrvatskoj je također u porastu. Čimbenici rizika za nastanak kolorektalnog karcinoma su životna dob, prehrambene navike, genetska sklonost i upalne bolesti crijeva. Nova saznanja o prognostičkim činiteljima su od velike važnosti u ranom otkrivanju i suvremenom pristupu liječenju pacijenata oboljelih od kolorektalnog karcinoma. Istraživanje provedeno na 10 pacijenata koji su liječeni na Klinici za kirurgiju KBB Rijeka pokazalo je da se karcinom debelog crijeva češće javlja u muškaraca (80%) i osoba starije dobe (od 66.-75. godine). Adenokarcinomi predstavljaju najčešći tip tumora stadija B i C prema Dukesovoj klasifikaciji. Patohistološka svojstva tumora su povezana sa širenjem tumora i utiču na ishod bolesti. Kolorektalni karcinomi stadija C prema Dukesovom sustavu imaju lošiju prognozu u odnosu na karcinome stadija B. Smješteni su na lijevom dijelu debelog crijeva, pokazuju slabiju diferenciranost, zahvaćaju regionalne limfne čvorove i imaju jači izražaj proliferacijskog biljega, Ki67. Uvođenje novih prognostičkih biljega od iznimne je važnosti za točnije predviđanje razvoja bolesti, a sukladno tomu i personaliziranom pristupu liječenja karcinoma debelog crijeva.

Colorectal cancer is the most common cancer and one of the leading causes of cancer-related mortality in the world. The incidence of colorectal cancer has been on the rise in Croatia. Risk factors for developing of colorectal cancer are age, dietary habits, genetic suspecribility and inflammatory bowel disease. New knowledge about prognostic factors are of great importance in early detection and modern approach to the treatment of patients with colorectal cancer. A study of 10 patients who were treated at the Department of Surgery, Clinical Hospital Rijeka showed that colorectal cancer is occured more frequently in males (80%) and in older adults (from 66.-75. years). Adenocarcinomas comprise the vast majority of the tumor with stages B and C according to Dukes classification. Histopathological features of colorectal cancers are associated with the spread of tumors and affect its outcome. Patients with tumor of Dukes' stage C have a poor prognosis than patients with tumor of Dukes' stage B. They are left sided, poorely differentiated, affecting the regional lymph nodes and have a strong expression of the proliferation marker, Ki67. The introduction of new prognostic markers is extremely important to accurately predict disease risk and consequently introducing personalized medicine in colorectal cancer treatment.
