Abstract
Karcinom mokraćnog mjehura je najčešći maligni tumor urinarnog trakta, s medijanom pojavljivanja između 50 i 70 godina. Rizik od karcinoma je veći u urbanim nego u ruralnim područjima. Neke industrije su posebno povezane s karcinom mokraćnog mjehura. To su kemijska industrija, industrija boja, lakova, guma i ulja, te plinska industrija. Karcinom mokraćnog mjehura zbog terapijskih, prognostičkih i didaktičkih razloga dijelimo na nemišićno invazivni i mišićno invazivni karcinom. Pri prezentaciji karcinoma mjehura većina su nemišićno invazivni karcinomi. Radikalna cistektomija je standarna procedura liječenja mišićno invazivnih karcinoma mjehura T2-T4a, N0-NX, M0. Ona je usko povezena s problemom urinarnih derivacija. Derivacije urina su kirurške metode kojima se napušta dio uropoetskog puta uz nemogućnost rekonstrukcije te se urotrakt završava na neprirodnim, kirurški stvorenim otvorima. Tijekom posljednjih deseteljeća, ileum konduit je najčešće primjenjivana derivacija urina Ona predstavlja najbržu, najjednostavniju i komplikacijama najmanje sklonu derivaciju urina. Danas, zajedno s kontrolom raka, važno je očuvati kvalitetu života i sliku tijela bolesnika konstrukcijom kontinentog ortotopnog mjehura. Ortotopni mjehur ima neke tipične karakteristike normalnog mjehura, kao npr. mehanizam kontinencije, dovoljan kapacitet pri niskom intravezikalno tlaku te kompetentan antirefluksni mehanizam koji sprječava dilataciju gornjeg urinarnog trakta

Urinary bladder cancer is the most frequent malignant tumour of the urinary tract, mostly occurring between the age 50 and 70. The risk in the urban areas is greater than in the rural areas. Some industries are specifically connected with the urinary bladder cancer. They are chemical, paint, varnish, rubber, oil and gas industries. Bladder cancer for therapeutic, prognostic and didactic reasons, is divided into non-muscle invasive and muscle invasive cancer. At the presentation of bladder cancer most are muscle-invasive cancers. Radical cystectomy is the standard procedure for the treatment of muscle-invasive bladder cancer T2-T4a, N0-NX, M0. Urinary diversions are surgical methods that leaves part of uropoetic way with the impossibility of reconstruction, and urinary tract ends in unnatural, surgically created openings. During the last decades, ileal conduit has been the most common treatment for urinary diversion. It represents the fastest, easiest, least complication-prone urinary derivation. Nowadays, together with cancer control, it is important to preserve the quality of life and patients’ body image by constructing continent bladder substitute. An orthotopic neobladder possesses some typical characteristics of the normal bladder, like continence mechanism, sufficient capacity at a low intravesical pressure and an antireflux mechanism competent to prevent dilatation of the upper urinary tract.
