Abstract
Deficit vitamina B12 je prilično često stanje koje se prezentira s hematološkim, probavnim i neuropsihijatrijskim simptomima i znakovima. Uzroci manjka vitamina B12 su loša iskoristivost, poremećena apsorpcija i nedostatna prehrana. Vitamin B12, još poznat pod nazivom cijanokobalamin, je uključen u metabolizam aminokiselina, popravak mijelina i stvaranje stanica koštane srži, te iz tih uloga proizlaze razne manifestacije koje nastaju ako ga nema dovoljno. Megaloblastična anemija je najčešća hematološka prezentacija. Neurološke komplikacije mogu ići sa ili bez simptoma megaloblastične anemije. Najčešća neurološka klinička manifestacija nedostatka vitamina B12 je subakutna kombinirana degeneracija koja je uzrokovana degeneracijom lateralnih i dorzalnih kolumni kralježnične moždine i neuropatijom. Neki bolesnici se prezentiraju s kognitivnim disfunkcionalnostima. Oni su razdražljivi i depresivni, često dementni, dok se manje često viđa slika psihoze. Dijagnoza se zasniva na laboratorijskim pretragama, razinama vitamina B12 u serumu i slikovnoj dijagnostici. Terapija je jednostavna primjena vitamina B12, ali dugotrajna i često doživotna

Vitamin B12 deficiency is a common cause of hematologic, gastrointestinal and neuropsychiatric signs and symptoms. Causes of vitamin B12 deficiency include poor utilization, inadequate intake and impaired absorption. Vitamin B12, known as cyanocobalamin, is involved in amino acid metabolism, myelin repair and bone marrow cell production. Therefore, impairment of these important roles results in main signs and symptoms of vitamin B12 deficiency. Megaloblastic anemia is the most common hematologic manifestation. Neurological complications can be accompanied with or without symptoms of megaloblastic anemia. Furthermore, the most common neurological clinical manifestation is subacute combined degeneration caused by degeneration of lateral and dorsal columns of spinal cord and neuropathy. Moreover, some patients present with cognitive impairment. They are fretful and depressive, often demented, while psychosis is not seen very often. Diagnosis is based on laboratory assessment, measurement of serum vitamin B12 levels and diagnostic imaging. Treatment includes simple administration of vitamin B12, but it is longlasting and often lifelong.
