Abstract
Gastropareza jest je sindrom u kojem je prisutno objektivno usporeno pražnjenje želuca u odsutstvu mehaničke opstrukcije. Glavni simptomi gastopareze su: mučnina, povraćanje, rana sitost, postprandijalna nadutost i epigastrična bol. Najčešći je uzrok gastropareze nepoznat (idiopatska), ali naučestalija poznata etiologija jest šećerna bolest (dijabetes mellitus, DM) te se procijenjuje da zahvaća jednu trećinu gastropareza. 5-12% oboljelih od DM-a imaju dijabetičku gastroparezu (DG), a primjećuje se njezin porast u svjetskoj populaciji radi sve većeg broja oboljelih od šećerne bolesti u svijetu. Zahvaća više žene nego mouškarce. 25 Smatra se da je centar mehanizma nastanka bolesti gubitak intersticijskih Cajalovih stanica i poremećaj enteričnog živčanog sustava zbog imuloške disregulacije. Veliku ulogu u patogenezi ima i autonomna neuropratija, kao jedna od kroničnih komplikacija šećerne bolesti. Pojava simptoma DG-e započinje otprilike nakon 10 godina od postavljanja dijagnoze DM-a i ima sklonost da perzistira usprkos optimalnoj glikemijskoj kontroli. Najdominantniji simptom gastropareze u DM-a jest mučnina. Zlatni standard u dijagnosti DG-e predstavlja scintigrafija pražnjenja želuca. Prva linija liječenja gastropareze jest promjena prehrane, optimalna glikemijska kontrola i upotreba prokinetika i antiemetika. U pacijenta u kojih gastropareza značajno umanjuje kvalitetu života , a nije učinkovita prva linija liječenja, onda su izbor invazivne metode poput električne stimulacije želuca. Cilj terapije je smanjiti broj hospitalizacija i poboljšati kvalitetu života i funkcionalni status u oboljelih.
Abstract (english)
Gastroparesis is a syndrome in which there is an objectively slow gastric emptying in the absence of mechanical obstruction. The main symptoms of gastoparesis are: nausea, vomiting, early satiety, postprandial bloating and epigastric pain. The most common cause of gastroparesis is unknown (idiopathic), but the most common known etiology is diabetes (diabetes mellitus, DM) and it is estimated to affect one third of gastroparesis. 5-12% of DM patients have diabetic gastroparesis (DG), and its increase in the world population is applied due to the growing number of diabetics in the world. It affects more women than men. The center of the disease mechanism is thought to be the loss of interstitial Cajal cells and enteric nervous system disorders due to immune disregulation. Autonomic neuropathy also plays an important role in the pathogenesis, as one of the chronic complications of diabetes. The onset of DG symptoms begins approximately 10 years after DM is diagnosed and tends to persist despite optimal glycemic control. The gold standard for diagnosing DG is gastric emptying scintigraphy. The first line of treatment for gastroparesis is a change in diet, optimal glycemic control, and the use of prokinetics and antiemetics. In patients in whom gastroparesis significantly reduces the quality of life, and the first line of treatment is not effective, then invasive methods such as electrical stimulation of the stomach are the choice. The goal of therapy is to reduce the number of hospitalizations and improve the quality of life and functional status in patients.
