Abstract
Crohnova bolest je sustavna, kronična granulomatozna bolest koje može zahvatiti bilo koji dio probavnog sustava. Dijagnoza se uglavnom postavlja kombinacijom kliničkih znakova, biokemijskih pretraga, endoskopskih, radioloških i histoloških nalaza. Endoskopski se može pregledati probavni sustav do duodenuma i terminalnog ileuma. Video kapsulska endoskopija (VCE) se pojavila početkom 21. stoljeća kao nova dijagnostička metoda. Primarna ideja ove metode je bila kompletan pregled tankog crijeva na način da se kapsula s izvorom svjetlosti i kamerom proguta i snima slike koje kasnije gastroenterolog pregledava. VCE je metoda koja ima prednosti i mane. Trenutno se preporuča korištenje VCE kao prvi izbor u dijagnostici anemija nejasnih uzroka i krvarenja probavnog sustava kada uzrok nije nađen provedenim klasičnim endoskopskim metodama. Kod Crohnove bolesti preporuča se učiniti nakon učinjene ileokolonoskopija koja nije u potpunosti dokazala postojanje bolesti, u kombinaciji s pozitivnim kliničkim znakovima koji upućuju na potencijalnu Crohnovu bolest. Postoje određeni rizici i kontraindikacije za korištenje VCE kao što su retencija kapsule kod prije poznatih striktura, iako u malom postotku. Analizom troškova još nije moguće sa sigurnošću tvrditi da je VCE isplativa metoda dijagnostike, ali istraživanja zasada pokazuju pozitivna predviđanja. S razvojem novih modela kapsula i sustava bodovanja, vizualizacija će a samim time i mogućnost potvrđivanja lezija biti kvalitetnija, a dijagnoza točnija.

Crohn's disease is a systemic, chronic granulomatous disease that can effect any part of the gastrointestinal tract. Diagnosis of Crohn's disease is usually made by a combination of clinical, biochemical, endoscopic, radiological and histologic findings. Conventional endoscopy can detect involvement up to duodenum and terminal ileum. Video capsule endoscopy (VCE) appeared in the beginnings of the 21st century as a new diagnostic method. The primary idea of this diagnostic tool was complete assessment of small bowel by ingesting a capsule which involved a camera and a source of light, which takes photos of the intestines that are later reviewed by a gastroenterologist. At the moment VCE is recommended as a first line of diagnostics in anemias caused by obscure gastrointestinal bleeding and obscure bleeding after classical endoscopic diagnostic was performed inconclusive. Regarding Crohn's disease, VCE is recommended after an inconclusive ileoclonoscopy, followed by positive clinical signs that are pointing to potential Crohn's disease. There are certain risks and contraidications in using VCE such as capsule retention with previously positive strictures, although in a small percentile of cases. Cost-effective analysis still can not confirm if VCE is the best diagnostic tool, but so far the research suggest positive predictions. With the development of the new capsule models and better scoring systems, the visualisation and the ability to diagnose lesions will improve, making the diagnostics more accurate
