Abstract
Unutarnja resorpcija korijena je patološko stanje pulpe karakterizirano progresivnim oštećenjem intraradikularnog dentina uslijed resorptivnog djelovanja upaljenog granulacijskog tkiva. Etiologija nastanka unutarnje resorpcije zuba nije u potpunosti razjašnjena dok se u literaturi kao uzrok najčešće spominje kronična upala pulpe i trauma. Ostali uzroci su jatrogene prirode ili se radi o idiopatskoj resorpciji korijena. Uslijed djelovanja nekog etiološkog čimbenika dolazi do biokemijskih promjena i diferencijacije mezenhimalnih stanica u odontoklaste. Klastičnim djelovanjem orijaških stanica nastaju resorptivne lakune a, dentin se dodatno otapa zbog sniženja pH vrijednosti i pritiska granulacijskog tkiva na dentinske stjenke. Napredovanjem procesa dolazi do nekroze pulpe i resorpcija se zaustavlja. Moguće komplikacije su perforacija dentinskih zidova te napredovanje infekcije u parodontni prostor i nastanak apikalnog parodontitisa. Dijagnoza se postavlja na temelju anamneze, kliničkog nalaza i radioloških snimki. Važno je zabilježiti prethodne simptome zuba i provedene terapijske postupke, kao i povijest traume i provedene ortodontske terapije. Kliničkim pregledom je moguće utvrditi simptome kroničnog pulpitisa i primijetiti izmijenjenu boju kliničke krune. Ipak, najčešće se radi o asimptomatskom procesu koji se otkriva slučajno na rutinskoj snimci. Radiolucencija korijenskog kanala je karakterističnog oblika sa ovalnim proširenjem. Terapijske mogućnosti zbrinjavanja resorpcije korijena su endodontska terapija, kirurška resekcija korijena i ekstrakcija.

Internal root resorption is a pathological condition characterized by progressive damage to the intraradicular dentin due to resorptive activity of inflamed granulation tissue. Etiology of internal resorption is not entirely clear, as the most frequently mentioned cause is chronic inflammation of the pulp and trauma. Other causes are iatrogenic or idiopathic. The action of an etiological factor leads to biochemical changes and differentiation of mesenchymal cells into odontoclasts. Due to clastic action of giant cells, resorption lacunaes are formed. The dentin is further dissolved by reduced pH and pressure of granulation tissue. Advanced process leads to pulp necrosis and resorption is stoped. Possible complications include perforation of the dentin wall, progression of the infection into the periodontal space and the emergence of apical periodontitis. Diagnosis is based on anamnesis, clinical findings and radiological images. It is important to note the symptoms and therapeutic procedures performed, as well as a history of trauma and implemented orthodontic treatment. Clinical examination is possible to identify the symptoms of chronic pulpitis and to notice the changed color of the clinical crown. However, most often it is a silent process that is revealed on a routine radiological image. Radiolucence of the root canal is of characteristic shape with an oval enlargement. Therapeutic choices are endodontic therapy, surgical resection and tooth extraction.
