Abstract
Karcinom debelog crijeva i rektuma u današnje vrijeme zbog rastuće incidencije predstavlja sve veći dijagnostički i terapijski izazov. U današnje vrijeme suvremene medicine laparoskopska kirurgija postaje standardom u brojnim svjetskim centrima, tako i u kirurškom liječenje kolorektalnog karcinoma. Cilj ovog istraživačkog rada je usporediti laparoskopski i klasični pristup anteriorne resekcije rektuma kod karcinoma u Kliničkom bolničkom centru Rijeka. Ovom retrospektivnom studijom obuhvaćeno je 54 bolesnika liječenih na Zavodu za digestivnu kirurgiju, Klinike za kirurgiju Kliničkog bolničkog centra Rijeka u razdoblju od 1.siječnja 2013. do 31.prosinca 2013. godine. Klasičnim pristupom operirano je 23 pacijenata, dok laparoskopskim pristupom je operirano 31 pacijent. Oporavak pacijenata koji su operirani minimalno invazivnom kirurgijom je mnogo brži i već treći dan postoperativno su sposobni za samostalnu njega. Trajanje postoperativnog ileusa je skraćeno i prva stolica se pojavljuje 4. dana što je brže nego kod pacijenata koji su operirani klasičnim pristupom,8.dan. Brži oporavak rezultira kraćem postoperativnom boravku u bolnici. Pacijenti koji su operirani laparoskopskim pristupom otpušteni su na kućnu njegu 5. dana, dok pacijenti operirani klasičnim pristupom otpušteni su tek 10. dan. Kardiovaskularne komplikacije i infekcija rane su postoperativne komplikacije koje se statistički značajnije manje pojavljuju kod pacijenata koji su laparoskopski operirani. Vremensko trajanje operativnog zahvata između dvije skupine ispitanika nema statistički značajne razlike.

Because of growing incidence last few years, colorectal cancer is diagnostic and therapeutic challenge. Laparoscopic surgery is becoming the standard in modern medicine, including the surgical treatment of colorectal cancer. The aim of this research study is the comparison of laparoscopic and classical approach to the anterior resection of rectal and sigma cancer in Clinical Hospital Centre of Rijeka. This retrospective study included 54 patients treated at the Department of Digestive surgery in Clinical Hospital Centre of Rijeka, in period from 1. January to 31. December 2013. Classical surgery was performed in 23 patients, while the laparoscopic approach in 31 patients. Patients treated with minimally invasive surgery are much faster recovered, and on the third postoperative day they are capable for self-care. The duration of post-operative ileus is shortened and the first stool appears at the fourth day. That is faster than in patients who were treated with classical approach (8 days). The group of patients who were treated with laparoscopic approach were discharge from hospital at the fifth day, and another group were discharge on the tenth day. Cardiovascular complications and wound infection were postoperative complications, which were statistically significantly less prevalent in laparoscopic surgery. Duration of surgery treatment between the two groups of patients was without significant difference.
