Abstract
Bubrežno zatajenje je najteži oblik bubrežne bolesti. Najčešći uzrok nastanka bubrežnog zatajenja je arterijska hipertenzija, šećerna bolest i glomerulonefritis. Progresijom bubrežnog zatajenja nastaju promjene na razini cijelog organizma. U 90% slučajeva zahvaćena je usna šupljina. Vide se promjene na sluznici, zubima, parodontu i čeljusnim kostima. Bljedoća sluznice, uremički zadah, suhoća usta, sklonost infekcijama i krvarenju, hiperplazija gingive, bubrežna osteodistrofija i uremički stomatitis neke su od posljedica bubrežnog zatajenja i njegova liječenja. Sveobuhvatno liječenje bolesnika s bubrežnim zatajenjem treba obuhvaćati i liječenje u ordinaciji dentalne medicine. Bolesnici trebaju biti osviješteni o važnosti prevencije oralnih manifestacija. Doktor dentalne medicine mora poznavati prilagodbu lijekova i potrebu za antibiotskom profilaksom prije invazivnih dentalnih zahvata. Također, doktor dentalne medicine treba biti dio multidisciplinarnog tima za nadzor i liječenje bubrežnog zatajenja i njegovog utjecaja na organizam. Suradnja s nefrologom je neophodna.
Abstract (english)
Renal failure is the most severe form of kidney disease. The most common causes of kidney failure are arterial hypertension, diabetes mellitus and glomerulonephritis. Progression of renal failure resulting changes at the level of the entire organism. In 90% of cases the oral cavity is affected. Changes in the oral cavity can be seen on the oral mucosa, teeth, periodontium and jaw bones. Paleness of the mucous membranes, uremic breath, dry mouth, susceptibility to infections and bleeding, gingival hyperplasia, renal osteodystrophy and uremic stomatitis are some of the manifestations of kidney disease and its treatment. Comprehensive treatment of patients with renal failure should include the treatment in dental office. Patients should be aware of the importance of prevention of oral manifestations. Dentist must know the adjustment of drugs and the need for antibiotic prophylaxis before invasive dental treatment. Also, the dentist should be part of a multidisciplinary team for the control and treatment of renal failure and its impact on the body. Cooperation with the nephrologist is mandatory.
