Abstract
Umnjak je zub koji je na brojne načine drugačiji od ostalih zubiju u čeljusti, zadnji je zub koji niče i stoga mu je na raspolaganju najmanje prostora za pravilan rast i upravo zbog toga ima učestalost impakcija od 33%. Osim manjka raspoloživog prostora uzroci impakcije su brojni. U literaturi, ali i u svakodnevnom kliničkom radu navodi se velik broj komplikacija vezanih uz impaktirane zube. Perikoronitis je stanje koje najčešće i to čak u preko 90% slučajeva povezujemo sa impaktiranim donjim kutnjacima. Na temelju iscrpne anamneze, odgovarajućeg ekstraoralnog i intraoralnog pregleda te analize radioloških snimaka kliničar donosi individualni plan terapije. Radiološka snimka važan je korak u planu terapije budući da otkriva položaj, oblik i broj korijenova što je važno za odabir operativnog pristupa. Važno je držati se navedenih indikacija i kontraindikacija za vađenje umnjaka. Alveolotomija je najčešći oralnokirurški zahvat kojim se uklanja koštani dio zubne alveole koji nadkriva impaktirani zub, nakon čega slijedi ekstrakcija zuba koji se potpuno formirao u kosti, ali nije iznikao na svojem mjestu, ni na bilo kojem mjestu u zubnom luku ili izvan njega.

Wisdom tooth is a tooth that is in many ways different from other teeth in the jaw, it is the last tooth that erupts and therefore it has the least amount of space for proper eruption and it is the main reason why impaction has a frequency of 33%. Besides the lack of available space causes of impaction are numerous. In literature, but also in everyday clinical practise there is a big number of complications associated with impacted teeth. Pericoronitis is a condition that is most often and even in 90% of cases associated with impaction of lower third molars. Based on a thorough medical history, a suitable extraoral and intraoral examination and the analysis of radiological images clinician brings an individual treatment plan. Radiograph is an important step in the plan of therapybecause it shows the position, shape and number of the roots of what is imporant to select the surgical approach. It is important to keep the above mentioned indications and contraindications for extraction of wisdom teeth. Alveolotomy is the most common oral – surgery that removes part of the dental alveolar bone which shelters impacted tooth after that follows the extraction of teeth that are fully formed in the bone, but has not erupted in their place, nor in any other place in the dental arch or outside.
