Abstract
Hemostaza je spontani prestanak krvarenja iz oštećene krvne žile. Trombociti zauzimaju središnje mjesto u hemostazi. Tromboza je patološko stanje u kojem nastaje tromb u odsutnosti krvarenja. Ozljeda krvne žile, promijenjeni protok krvi ili abnormalna koagulabilnost krvi su glavni uzroci tromboze. Ako tromb biva otkinu sa mjesta nastanka, postaje embolus, te putem krvi može biti raznesen po cijelome tijelu. Antitrombocitni i antikoagulantni lijekovi predstavljaju skupinu ljekova koja se koristi u prevenciji i terapiji tromboembolijskih bolesti. Antitrombocitni lijekovi interferiraju sa trombocitnom funkcijom na način da inhibiraju trombocitnu agregaciju. Antikoagulansi inhibiraju proizvodnju faktora zgrušavanja krvi. Proteklih petnaestak godina u svakodnevnom dentalnom radu i prilikom oralno-kirurških zahvata pacijenti su uglavnom prekidali svoju terapiju protiv zgrušavanja krvi 7-10 dana prije zahvata. Novija istraživanja su pokazala da to može uzrokovati tromboembolijske komplikacije u postoperativnom razdoblju. S druge strane, niti jedno istraživanje nije dokazalo veći rizik od hemoragijskih komplikacija kod pacijenata koji su nastavili antitrombocitnu ili antikoagulantnu terapiju za vrijeme oralno-kirurških i dentalnih zahvata. Zaključak novijih studija je da pacijente koji su na antitrombocitnoj ili antikoagulantnoj terapiji ne treba skidat sa terapije prije dentalnih i jednostavnijih oralno-kirurških zahvata.

Hemostasis is a spontanius way to stop the bleeding from injured blood vessel. Platelets takes the central place in hemostasis. Thrombosis is a patological condition in which thromb is created in absence of bleeding. Blood vessel injury, recreated blood flow or abnormal blood coagulation are the most common reasons of trombosis. When thromb is detached from a place where it appearce it becomes embolus and by blood stream it may be carried away to another part of the body. Antiplatet and anticoagulant medications represent the group of drugs that are used in the prevention and treatment of tromboembolic diseases. Antiplatet medications interfere with the functioning of platelets, thus inhibiting platelet aggregation. Anticoagulants inhibit the production of clotting factors. In the last fifthteen years in daily dental treatment or oral surgery patients were mostly discontinuing their antiplatet and anticoagulant therapy 7-10 days before treatment. New studies have proven that discontinuing the therapy may cause thromboembolic complications during the postoperative period. On the other hand, there is no study that proves enlarged risk of hemorragic complications acording to patients who continued their antiplatelet or anticoagulant therapy during oral surgery and dental treatments. New studies concluded that their is no need to discontinue antiplatelet or anticoagulant therapy before daily dental treatment or oral surgery.
