Abstract
Tijekom proteklih 15 godina, implantologija je uznapredovala brže od mnogih drugih disciplina dentalne medicine. Estetika je postala neodvojivi dio oralne rehabilitacije te pacijenti ne očekuju samo da protetska rehabilitacija na implantatima bude funkcionalno dugoročna, već i da estetski zadovoljava posebice u regiji u usnoj šupljini koja je vidljiva kada se pacijent nasmiješi. Liječenje visokorizičnih bolesnika identificiranih kroz detaljnu preoperativnu analizu i opću procjenu rizika (zdravstveno stanje, parodontna predispozicija, pušenje i drugih rizici) treba provoditi s oprezom, budući da su estetski rezultati manje dosljedni. Naime, ugradnja implantata u prednjoj čeljusti smatra se složenim postupkom i zahtijeva sveobuhvatnu analizu prije zahvata, detaljno planiranje i korištenje kirurškog protokola koji zagovara postavljanje implantata vođeno nadomjestkom. Precizna trodimenzionalna ugradnja implantata najbitniji je čimbenik za ishod estetskog liječenja. Poštivanje zone udobnosti u trima dimenzijama rezultira idealnom pozicijom ramena implantata, stvarajući time povoljne uvjete za postavu protetske suprastrukture okružene dugoročno stabilnom periimplantatnom mukozom. Za stabilnost preiimplantatnog tkiva potreban je dovoljan horizontalni i vertikalni volumen kosti. Kada postoje takvi nedostaci potrebno je provesti specifične procedure, uključivši i nove protokole augmentacije kosti, transplantiranje vezivnog tkiva i rekonstrukcija izgubljenih papila. Kako bi ishod liječenja bio optimalan, preporučljivo je korištenje privremenih nadomjestaka s odgovarajućim izlaznim profilom za vođenje i oblikovanje periimplantatnog tkiva prije postave konačnog nadomjestka. Predvidljivi optimalni rezultati u estetskoj regiji postižu se samo putem primjene sveobuhvatnog koncepta na temelju kliničkog iskustva, preoperativne obrade i planiranja uz timski pristup koji ujedinjuje pacijenta, specijalista kirurgije, protetike i dentalnog tehničara.
Abstract (english)
Over the past 15 years, implant dentistry has progressed faster than many other disciplines in dental medicine. Esthetics has become an inseparable part of oral rehabilitation as patients not only expect implant-supported restorations to be functional long-term, but also to be esthetic, especially in regions of the oral cavity that are visible when tha patient smiles. Implant therapy in the anterior maxilla is considered an advanced or complex procedure and requires comprehensive preoperative planning and precise surgical execution based on a restoration-driven approach. Treatment of high-risk patients identified through site analysis and a general risk assesment (medical status, periodontal susceptibility, smoking, and other risks) should be undertaken with caution, since esthetic results are less consistent. Correct three-dimensional implant placement is esential for an esthetic treatment outcome. Respect of the comfort zones in these dimensions results in an implant shoulder located in an ideal position, allowing for esthetic implant restoration with stable, long-term peri-implant tissue support. For long-term esthetic soft-tissue stability, sufficient horizontal and vertical bone volume is essential. When deficiences exist, appropriate hard and soft-tissue augmentation procedures are required. To optimize ethetic treatment outcomes, the use of provisional restorations with adequate emergence profiles is recommended to guide and shape the peri-implant tissue prior to definitive restoration. Nevertheless, predictable optimum results in the esthetic region can only be achieved through application of a comprehensive clinical concept based on experience, sound pre-operative examination and treatment planning, and a team approach that unites patients, surgeons, prosthodontists, and dental technicians.
