Abstract
Tumori žlijezda slinovnica pripadaju histološki raznolikoj skupini tumora glave i vrata te uključuju dobroćudne i zloćudne tumore. Najčešće sijelo tumora je parotidna žlijezda, a rijeđe submandibularna, sublingvalna i ostale manje žlijezde slinovnice. Tumori parotidne žlijezde većinom su benigni, no u 25% slučajeva javljaju se zloćudni tumori. Ultrazvučni pregled uz citopunkciju je glavna dijagnostička metoda za postavljanje dijagnoze tumora žlijezda slinovnica. Najčešći benigni tumor žlijezda... More slinovnica je pleomorfni adenom, dok su Whartinov tumor, adenom bazalnih stanica i adenom kanala žlijezda slinovnica nešto rijeđi. Primarni vid liječenja uključuje kirurško odstranjenje dijela ili cijele žlijezde slinovnice. Benigni tumori nakon adekvatnog kirurškog liječenja imaju odličnu prognozu. Ovaj rad je obuhvatio 70 ispitanika hospitaliziranih i operiranih na Klinici za maksiofacijalnu kirurgiju pri KBC-u Rijeka u razdoblju od 1. siječnja 2015. godine do 31. prosinca 2017. godine. Svrha ovog rada je bila prikazati pojavnost tumora žlijezda slinovnica s obzirom na dob, spol, lokalizaciju, citološki nalaz i patohistološki nalaz te utvrditi osjetljivost i specifičnost citopunkcije u sklopu prijeoperativne dijagnostike. Rezultati ovog istraživanja, pokazuju kako se dobroćudni tumori žlijezda slinovnica češće javljaju u odnosu na zloćudne tumore te kako tumori žlijezda slinovnica najčešće zahvaćaju parotidnu žlijezdu. Pojavnost tumora s obzirom na spol, stranu lica na kojoj se razvio tumor te konzumaciju duhanskih i alkoholnih proizvoda, nije statistički značajna. Ultrazvučno vođena citopunkcija pokazala je više benignih tumora u odnosnu na maligne tumore. Pacijenti su najčešće liječeni suprafacijalnom parotidektomijom. Pleomorfni adenom i Warthinov tumor su najčešće patohistološke dijagnoze. UTZ vođena citopunkcija je pokazala visoku osjetljivost i specifičnost u sklopu prijeoperativne dijagnostike te pruža mogućnost ispravnog planiranja operativnog zahvata. Less

Salivary gland tumors comprise a histologically diverse group of head and neck tumors, which can be either benign or malignant. The most common tumor site is the parotid gland; the remainder originate in the submandibular, sublingual, and minor salivary glands. Parotid gland tumors are mostly benign, but in 25% of cases malignant tumors occur. Diagnosis is based on ultrasound guided fine needle biopsy. The most common benign tumor of the salivary glands is pleomorphic adenoma, while Whartin... More tumor, basal cells adenoma, and canalicular adenoma appear less frequently. The primary form of treatment involves superficial or total surgical removal of the salivary gland. Benign tumors have excellent prognosis after adequate surgical resection. This survey included 70 patients treated at the Maxillofacial Surgery Clinic at Clinical Hospital Centre Rijeka in the period from 1 January 2015 to 31 December 2017. The purpose of this study was to show the incidence of the salivary gland tumors by age, sex, localization, cytological and histological findings. Another aim of this study was observing both specifity and sensitivity of the ultrasound guided fine needle biopsy. According to the results of this study, benign tumor of the salivary glands are much more frequent in comparison to malignant tumors and most commonly affect the parotid gland. In this study, the effect on incidence of tumors by sex, alcohol and smoking has not shown statistically significant difference. Superficial parotidectomy was the most common method of treatment. There were much more benign tumors proved cytologically, while pleomorphic adenoma and Whartin tumor were the most commonly found tumors histologically. Ultrasound guided fine needle biopsy has shown high specifity and sensitivity in preoperative diagnostics and therefore it helps to properly plan surgical treatment. Less
