Abstract
Ne-sitnostanični karcinomi pluća (NSSKP) danas su izrazito učestali s prosječnim petogodišnjim preživljenjem od svega 15%. U ranim stadijima simptomi ovih tumora su blagi i često se dijagnosticiraju u uznapredovalom stadiju kada je uspješnost standardne terapije vrlo ograničena. Posljednjih godina se iz navedenog razloga sve više istraživanja usmjerava prema ciljanoj terapiji usmjerenoj na mutirane EGFR, ALK i dr. receptore i enzime kao i na blokadu različitih imunoloških i neimunoloških interakcija unutar tumorskog mikrookoliša. Ligand programirane stanične smrti-1 (PD-L1) molekula je koja učinak ostvaruje kroz inhibiciju aktivacije i efektorske funkcije različitih imunoloških stanica, dominantno citotoksičnih T limfocita. S druge strane omogućava tumorskim stanicama da izbjegnu prepoznavanje od strane imunološkog sustava domaćina. Brojna istraživanja analizirala su prognostički značaj ekspresije ove molekule te su pokazala uspjehe terapije ciljanim inhibitorima PD-L1 liganda te njemu pridruženog PD-1 receptora. U našem istraživanju ekspresiju PD-L1 analizirali smo zasebno na tumorskim i intersticijskim stanicama te ju bilježili u vidu H-skora. U obzir je uzet postotak obojenih stanica i intenzitet obojenja. Statistička značajnost pokazana je u razlici ekspresije PD-L1 u različitim histološkim tipovima NSSKP-a. Također je dobivena statistički značajno povećana ekspresija PD-L1 na tumorskim i intersticijskim stanicama s kliničko-patološkim prognostičkim obilježjima NSSKP-a , točnije s gradusom tumora, patološkim i kliničkim stadijem bolesti, proliferacijskim indeksom tumora, te invazijom pleure. Ovakvi rezultati jasno impliciraju da je ekspresija PD-L1 na tumorskim i intersticijskim stanicama od prognostičke važnosti što je konzistentno s rezultatima drugih istraživanja u ovom području.
Abstract (english)
Non-small cell lung cancer (NSCLC) is common today with average five year survival rate of just 15%. In early phase, symptoms of these tumors are mild or absent, so they are often diagnosed in advanced stages when effects and success of the standard treatment is oftentimes limited and unsuccessful. Because of that, in recent years, more and more researches are aiming to new ways of treatment. The mutations of EGFR, ALK and other receptors and enzymes are utilized to treat patients but tumor microenvironment, with its different immunological and non-immunological interactions offers a wide spectrum of possibilities for targeted therapy. Programmed death ligand-1 (PD-L1) is a molecule which function is seen through inhibition of different immune cells, predominantly cytotoxic T-lymphocytes, so that tumorcan evade our immune system. Many researchers analysed the prognostic significance of PD-L1 expression on tumorand/or immune cells and showed the positive effects of targeted therapy guided toward blockage of the interaction between PD-L1 and its PD-1 receptor. In this paper we analysed the expression of PD-L1 on tumor and interstitial cells which was shown as H-score. Statistical significance was shown in correlation of PD-L1 expression and different histologic types of NSCLC. Positive correlation of PD-L1 staining on tumor and interstitial cells was associated with factors that influence the NSCLC prognosis as pathological grade of the tumor, higher pathological and clinical stage, proliferation index oftumor cells and pleural invasion. These results imply that expression of PD-L1 on tumor and interstitial cells has prognostic significance. This conclusion is consistent with researches regarding this theme.
