Abstract
Ciroza jetre je ireverzibilna kronična bolest jetre koju karakterizira oštećenje jetrenog tkiva te progresivno propadanje jetrene funkcije. Najčešći uzrok bolesti je pretjerana konzumacija alkohola. Razvoj i opseg alkoholnog oštećenja jetre ovisi o količini konzumiranog alkohola, načinu i duljini konzumacije, socijalnim uvjetima, spolu te o genetskim faktorima. Najnoviji dokazi upućuju na povezanost čimbenika nekroze tumora alfa (TNF-α) s nastankom ciroze jetre. Cilj ovog rada bio je istražiti utječe li polimorfizam TNF-α-308 na podložnost i razvoj alkoholne ciroze jetre. U istraživanju je sudjelovalo 123 bolesnika s alkoholnom cirozom jetre dok je kontrolnu skupinu činilo 246 zdravih ispitanika. Polimorfizam TNF-α-308 G u A utvrđivan je metodom lančane reakcije polimeraze nakon koje je slijedila restrikcija s restrikcijskim enzimom (Nco I) te provjera elektroforezom na agaroznom gelu. Frekvencija TNF-α-308 genotipova i alela nije se značajno razlikovala (p>0,05) između ispitanika s alkoholnom cirozom jetre i kontrolne skupine. Distribucija genotipova među spolovima također se nije značajno razlikovala (p>0,05). U radu su prikazani i biokemijski parametri (γ–GT, AST, alkalna fosfataza, albumini) koji se koriste pri dijagnozi alkoholne bolesti jetre. Njihove vrijednosti nisu se značajno razlikovale obzirom na genotip bolesnika. Na osnovi rezultata možemo zaključiti da polimorfizam-308 TNF-α gena nema ključnu ulogu u etiopatogenezi alkoholne ciroze jetre u naših ispitanika.

Cirrhosis is irreversible chronic liver disease characterized by damage of liver tissue, and progressive deterioration of liver function. The most common cause of the disease is excessive alcohol consumption. Development and level of alcoholic liver damage depends on the amount of alcohol consumed, the length of consumption, social status, gender, and genetic factors. Recent evidence suggests an association between tumor necrosis factor alpha (TNF-α) with the development of liver cirrhosis. The aim of this study was to investigate the influence of the polymorphism of TNF-α-308 on the occurrence of cirrhosis. The study included 123 patients with cirrhosis and the control group consisted of 246 healthy subjects. Polymorphism of TNF-α-308 G to A was determined by polymerase chain reaction followed by digestion with the restriction enzyme Nco I and checks by agarose gel electrophoresis. The frequency of TNF-α-308 genotypes and alleles did not differ significantly (p>0,05) between patients with alcoholic liver cirrhosis and the control group. The distribution of genotypes between the sexes also were not significantly different (p>0,005). The paper also presents some biochemical parameters (γ–GT, AST, alkaline phosphatase, albumin) that are used in the diagnosis of alcoholic liver disease. There was no significant correlation (p>0,05) between TNF-α-308 genotypes and biochemical parameters. Based on the results, we can conclude that the polymorphism of TNF-308-α gene doesnt have a key role in pathogenesis of alcoholic liver cirrhosis in our patients.
