Abstract
CILJ : Analizirati i usporediti kretanje incidencije i mortaliteta od raka dojke u žena u Primorsko goranskoj županiji i Gradu Zagrebu za period od 2001.do 2011. godine. METODE I MATERIJALI : U radu su korišteni podaci za incidenciju Registra za rak Hrvatskog zavoda za javno zdravstvo. Analizirani podaci obolijevanja žena od raka dojke prikazani su kao apsolutni brojevi i kao dobno specifične stope na 100.000 žena. Podaci o mortalitetu su prikupljeni iz baze podataka Hrvatskog zavoda za javno zdravstvo, te su prikazani kao apsolutni brojevi i kao dobno specifične stope na 100.000 umrlih žena. Korišteni su podaci o broju stanovnika prema popisu stanovništva iz 2001. i 2011. godine. Državnog zavoda za statistiku. Za usporedbu dvije populacije koristili smo metodu dobne standardizacije i podatke prikazali kao dobno standardizirane stope. REZULTATI : U Primorsko – Goranskoj županiji, od 2001. do 2011. godine, ukupno je od raka oboljelo 2205 žena. Tako je 2001. godine stopa incidencije za rak dojke u žena iznosila 123,6/100 000 dok je 2011. godine porasla na 130,0/100 000. Stopa incidencije kod žena raste s dobi. Veći porast dobno – specifične stope vidljiv je nakon 40. – te godine života. U 2001. godini dobno specifična stopa incidencije, za dob između 40. i- 44. godine života, iznosila je 189/100 000 žena, a najviša je u dobnoj skupini od 80 do 84. godina, 778/100 000 žena. Uočen je porast incidencije 2011. godine u dobi od 50 do 69 godina. U Gradu Zagrebu, od 2001. do 2011. godine oboljelo je, 5166 žena od raka dojke. Tako je 2001. godine stopa incidencije za rak dojke u žena iznosila 133/100 000 dok je 2011. godine pala na 100/100 000. Za 2001. godinu, dobno – specifična stopa incidencije iznosila je 103/100 000 žena, za dob između 40. i- 44. godine života. Usporedba dobno standardiziranih stopa incidencije raka dojke u žena PGŽ i Zagreba za dob 0 do 64 godine, pokazuje višu stopu incidencije u svim promatranim godinama u PGŽ osim od 0 – 64. Tako je 2011. godine dobno standardizirana stopa incidencije raka dojke iznosila 34,5/100 000 žena u Zagrebu, a 88,7/100 000 žena u u Primorsko – Goranskoj županiji. Incidencija u PGŽ u svim promatranim godinama pokazuje trend rasta, dok u Zagrebu pokazuje blagi trend smanjivanja. U razdoblju od 2001. do 2011. godine u Primorsko-Goranskoj županiji od raka dojke umrlo je 750 žena. U Primorsko – Goranskoj županiji analiza kretanja stopa mortaliteta raka dojke 2001. i 2010. godine je iznosila 40/100 000 žena. Stopa mortaliteta kod žena raste s dobi. Najviši mortalitet 2010. godine zabilježen je nakon 85. godine i iznosio je 642/100 000 žena. Grad Zagreb od 2001. do 2011. godine bilježi 1978 žena umrlih od raka dojke. Analiza kretanja stopa smrtnosti raka dojke u Gradu Zagrebu pokazuje trend pada 2011. godine i iznosi 43/100 000 žena. U dobi od 40 do 44 godine, dobno- specifična stopa je u 2001. godini iznosila 13/100 000 žena, dok je 2011.godine iznosila 7/100 000 žena. Usporedba dobno standardiziranih stopa mortaliteta raka dojke u žena Primorsko- Goranske županije i Grada Zagreba za dob 0 do 64 godine, pokazuje manju stopu mortaliteta u Gradu Zagrebu i 2011. godine iznosila je 9,9/100 000 žena dok je u Primorsko- Goranskoj županiji iznosila 21,1/100 000 žena. ZAKLJUČAK : Rak dojke spada u najčešće zloćudne bolesti koje zahvaćaju žene kako u razvijenim tako i u nerazvijenim dijelovima svijeta. Uzimajući u obzir rezultate koje smo dobili računajući stope incidencije i mortaliteta u Primorsko– Goranskoj županiji i Gradu Zagrebu uočeno je da stope incidencije i mortaliteta imaju trend porasta u obje županije, a vrlo vjerojatno i u cijeloj Hrvatskoj. Veća stopa incidencije prevladava u Primorsko – Goranskoj županiji, dok je u Gradu Zagrebu veća stopa mortaliteta. Znatnije povišenje incidencije u obje županije pojavljuje se nakon 50. godine u žena, a najviša stopa mortaliteta u obje županije prikazuje se nakon 80. godine u žena.

OBJECTIVE : To analyze and compare trends in the incidence and mortality of breast cancer in women in the Primorsko – Goranska County and the City of Zagreb for the period of 2001.do 2011. METHODS AND MATERIALS : We have used the data for the incidence of the Cancer Registry of the Croatian Institute for Public Health . The analyzed data, of developing breast cancer of women , are presented as absolute numbers and as age - specific rates per 100,000 women . Mortality data were collected from the database of the Croatian Institute for Public Health , and are presented as absolute numbers and as age - specific rates per 100,000 of the dead woman . We used the data on population according to the census of 2001. and 2011. central Bureau of Statistics . To compare the two populations we used a method of age standardization and display data as age- standardized rates . RESULTS: In the Primorsko – Goranska County, from 2001 to 2011, there were total of 2,205 cancer affected women. In 2001. the incidence rate for breast cancer in women was 123.6/100,000 while in 2011. it increased to 130,0/100 000. The incidence rate in women increases with age. The larger increase in age - specific rates is visible after 40 - years of age. In 2001, the age-specific incidence rate for the age between 40 and-44 years of age, was 189/100 000 women, and the highest in the age group of 80 to 84 years, 778/100 000 women. An increase in incidence in 2011. was for the age from 50 to 69 years. The city of Zagreb, from 2001. to 2011., 5166 women became ill from breast cancer. In 2001. the incidence rate for breast cancer in women was 133/100 000 while in 2011. it dropped to 100/100 000. For 2001., the age - specific incidence rate was 103/100 000 women in age between 40. and- 44 years. Comparison of age-standardized incidence rate of breast cancer in women PGŽ and Zagreb for age 0-64 years, showed a higher incidence rate in all the years under the PGŽ apart from 0 - 64. In 2011. the age-standardized incidence rate of breast cancer was 34.5/100,000 women in Zagreb, and 88.7/100 000 women in in Primorsko – Goranska County. The incidence of the PGž in all observed years shows an increasing trend, while in Zagreb, it shows a slight downward trend. In the period from 2001. to 2011. in PGŽ there was 750 death cases of breast cancer. In the Primorsko – Goranska County analysis of movement mortality rate of breast cancer in 2001. and 2010. was 40/100 000 women. The mortality rate among women increases with age. The highest mortality rate in 2010. was recorded after 85 years and was 642/100 000 women. The city of Zagreb from 2001. to 2011. recorded 1,978 women dying from breast cancer. Analysis of movement in the mortality rate of breast cancer in the City of Zagreb shows a downward trend in 2011. and it’s 43/100 000 women. At ages 40 to 44, age- specific rate in 2001. was 13/100 000 women, while in 2011. It was 7/100 000 women. Comparison of age-standardized mortality rate for breast cancer for women in PGŽ and the City of Zagreb for age 0-64 years, showed a lower mortality rate in the City of Zagreb and in 2011. was 9.9 / 100 000 women while in the PGŽ it showed 21.1/100,000 women. CONCLUSION: Breast cancer is one of the most common malignancy affecting women in both developed and undeveloped parts of the world. Taking into account the results we obtained, counting rate of incidence and mortality in the Primorsko- Goranska County and the City of Zagreb, it was observed that the rate of incidence and mortality showed an increasing trend in both counties, and quite possibly in whole Croatia. The higher incidence rate prevailing is in the Primorsko- Goranska County and in the City of Zagreb there is higher mortality rate. Significantly increase the incidence in both counties appears after age of 50, and the highest mortality rate in both counties is displayed after age of 80.
