Abstract
Maligni infarkt srednje moždane arterije uzrokuje povišenje intrakranijalnog tlaka i hernijaciju mozga. Zbog toga se smatra hitnim stanjem i zahtjeva liječenje u specijaliziranim jedinicama intenzivnog liječenja. Kardioembolija i tromboza srednje moždane arterije predstavljaju značajan uzrok ishemijskog moždanog udara. Dekompresivna kraniotomija i durotomija je kirurška metoda liječenja koja se koristi da se smanji povišenje intrakranijalnog tlaka i spriječi hernijaciju mozga koji se događaju unutar 28-48 sati od početka simptoma bolesti. Najvažniji pokazatelj stvaranja citotoksičnog edema i hernijacije mozga je zahvačenost područja srednje moždane arterije preko 50% što se najbolje prikazuje na CT-u mozga. Pravovremeno učinjena dekompresivna kraniotomija smanjuje smrtnost i pokazuje bolji ishod bolesti. Masovni i individualni pristup prevencije moždanog udara najbolji su načini za uvođenje promjene načina života i smanjenje razine čimbenika rizika u svih pojedinaca u populaciji. Rehabilitacija je sastavni dio liječenja oboljelih od moždanog udara. To je multidisciplinaran i stručno dobro organiziran proces koji za cilj ima kod svakog pojedinca postići što bolji oporavak i neovisnost.

Malignant middle cerebral artery infarction causes the elevation of the intracranial pressure and herniation of the brain. Therefore, it is considered a medical emergency and requires treatment in specialised intensive care units. Cardioemboli and thrombosis of middle cerebral arteries are an important cause of ischemic stroke. Decompresive craniectomy and durotomy is a surgical method of treatment that is used to reduce the elevation of intracranial pressure and prevent herniation of the brain that occur within 24-48 hours of onset of symptoms of the desease. The most important indicators of creating cytotoxic edema and herniation of the brain is area of involvement of middle cerebral artery over 50% of the territory wich is best displayed on CT of the brain. Timely done decompressive craniectomy reduces moratality and showed a better outcome. Massive and individual approach to prevent stroke are the best ways to itroduce lifestyle changes and redicing the level of risk factors in all individuals in the population. Rehabilitation is an integral part of the treatment of stroke patiens. It is a multidisciplinary and proffessional well-organized process that aims at each individual to achieve the greatest possible recovery and independence.
