Abstract
TMP-om nazivamo sva stanja kronične orofacijalne boli nedentalnog podrijetla. Predstavlja zajednički pojam koji obuhvaća brojna klinička stanja koja uključuju žvačne mišiće, čeljusne zglobove te okolne strukture. Funkcijska dijagnostika (klinička, manualna, instrumentalna analiza) omogućuje detaljniji uvid u funkcionalno stanje žvačnog sustava. U provođenju anamneze i kliničkog pregleda koristimo se upitnikom o povijesti bolesti koji ispunjavaju pacijenti te obrascem za kliničko ispitivanje temporomandibularnih poremećaja (Dijagnostički kriteriji za istraživanje temporomandibularnih poremećaja, DKI/TMP protokol). Ovo istraživanje je provedeno na Klinici za dentalnu medicinu Kliničkog bolničkog centra Rijeka i uključivalo je 32 ispitanika od čega je 16 bilo bolesnih, a 16 zdravih ispitanika. Uporabom standardiziranog DKI/TMP protokola prema osi I. utvrđeno je da ispitanici najčešće pate od mišićnog uzroka (16,4%), manje od artrogenog ( pomak diska - 1,6% te artralgija - 6,6%). Također su česte kombinacije više dijagnoza, s tim da čak 26,2% ispitanika ima trojnu dijagnozu (MP+PD+AAA). Kod pacijenata su prisutni bolovi i od strane mišića i zgloba prilikom palpacije u kretnjama otvaranja i zatvaranja čeljusti kao i prilikom ekstraoralne i intraoralne palpacije lica, glave i vrata te TMZ-a. Također su prisutni zvukovi u obliku škljocaja. Procjenom psihosocijalnog i funkcijskog statusa prema osi II. dokazujemo da pacijenti pate od kronične boli, više od polovice je doživjelo da im se čeljust uklješti te da im to ometa jedenje. Čeljusni problemi također onemogućavaju ili ograničavaju funkciju donje čeljusti, te je kod mnogih ispitanika prisutna pojava glavobolja i migrene. TMP omogućuje i nastajanje brojnih smetnji u obliku depresije i nespecifičnih fizikalnih simptoma.
Abstract (english)
Temporomandibular disorder is the name which refers to all chronic orofacial pain or of non dental origin. This disorder encompasses a number of clinical conditions. These clinical conditions are chewing muscles, jaw joints and surrounding structures. Functional diagnostics (clinical, manual, instrumental analysis) allows detailed insight into the functional state of the masticatory system. During anamnesis and clinical examination, we do further research and development on the history of the disorder and form clinical trial (Research Diagnostic Criteria for Temporomandibular Disorders, RDC / TMD protocol). This research was conducted at the department of dental medicine, University Hospital Center Rijeka and included 32 patients of whom 16 were sick and 16 healthy subjects. Using standardized RDC / TMD to the axis I found that respondents most often suffer from muscle causes (16.4%), less than joint causes (disc - 1.6%, arthralgia - 6.6%). There are often a combination of multiple diagnoses. 26.2% of respondents have a triple diagnosis (MD + PD + AAA). Patients have the pain from muscle and joint in the opening and closing movements of the jaw. Pain is present in extraoral and intraoral palpation of the face, head, neck and TMJ. Sounds are present in the form of a clicks. Assessing the psychosocial and functional status of the axis II. we prove that patients suffering from chronic pain. More than half had experienced clamp jaws and disrupts their eating. Jaw problems can also limit the functionality of the lower jaw. Many respondents suffer from headaches and migraines. TMP also allows the formation of numerous faults in the form of depression and nonspecific physical symptoms.
