Abstract
Vertikalna fraktura korijena je uzdužno orijentirana frakturalna pukotina koja se proteže od korijenskog kanala pa do okolnog parodontnog tkiva, najčešće u vestibulo-oralnom smijeru, a započinje u bilo kojem dijelu zuba, te može biti potpuna i nepotpuna. Vertikalna fraktura korijena uočena je kod endodontski liječenih zuba, ali i kod vitalnih zuba. Najčešće se javlja kod zuba sa opsežnim ispunima, endodontski liječenih zuba, zuba s opsežnim nadogradnjama i zuba opskrbljenih intrakanalnim kolčićima. Više čimbenika može uzrokovati vertikalnu frakturu korijena kod endodontski liječenih zuba a dijelimo ih u predisponirajuće i jatrogene čimbenike. Predisponirajući čimbenici su gubitak zdravog zubnog tkiva, anatomija korijena, gubitak vlage, ranije napuknuće u dentinu i gubitak okolne alveolarne kosti. Jatrogeni čimbenici su liječenje korijenskog kanala, preparacija i ugradnja intrakanalnog kolčića i neadekvatan izbor zuba kao nosača mosta. Klinički znaci VFK dijele se na nespecifične (koštani defekt, sinus trakt) i specifične (radiološka i kirurška dijagnostika). Terapijske mogućnosti su povezivanje frakturiranih fragmenata, resekcija, amputacija i ekstrakcija.
Abstract (english)
Vertical root fracture is a longitudinally oriented line that extends from root canal to the surrounding periodontal tissue, usually in the vestibule-oral direction, starts in any part of the tooth and may be complete or incomplete. Vertical root fracture was found in endodontically treated teeth, but also in vital teeth. Most common is in teeth with extensive restorations in endodontically treated teeth, teeth with extensive upgrades and teeth fitted with intracanal post. Several factors can cause vertical root fracture in endodontically treated teeth and divide them into predisposing and iatrogenic factors. Predisposing factors are the loss of healthy tooth structure, rootanatomy, moisture loss, earlier fracture in dentin and surrounding alveolar bone loss. Iatrogenic factors are treatment of root canal, preparation and installation of intracanal post and inadequate choice of teeth as the bridge girders. Clinical signs of vertical root fracture are divided into nonspecific (bone defect, sinus tract) and specific (radiological and surgical diagnoses). Therapeutic options are connecting the fractured fragments, resection, amputation and extraction.
