Abstract
Ne postoji jedna univerzalna definicija masivnog krvarenja. Najčešći uzroci masivnog krvarenja su trauma i veći elektivni kirurški zahvati. Značajan gubitak krvi dovodi do razvoja hemoragijskog šoka i koagulopatije. Hemoragijski šok je povezan sa smanjenom tkivnom perfuzijom, ishemijom, staničnom nekrozom i ako se ne liječi na vrijeme dovest će do višestrukog organskog zatajenja te smrti. Koagulopatija je multifaktorijalnog podrijetla i u kombinaciji s acidozom i hipotermijom formira ''smrtonosnu trijadu''. Kako bi mogli pratiti i identificirati promjene u bolesnikovom koagulacijskom sustavu vaţna je primjena testova kao što su PV, aPTV, mjerenje koncentracije fibrinogena, rotacijska tromboelastometrija i tromboelastografija. Kod liječenja vaţno je identificirati izvor krvarenja i uspostaviti odgovarajuću kontrolu krvarenja. Na početku se kao dio nadomjesne terapije primjenjuju infuzije kristaloidnih i koloidnih otopina. Kad se uspostavi potreba za krvnim pripravcima daju se u odgovarajućim omjerima transfuzije eritrocita, trombocita i plazme. U regulaciji koagulopatije koriste se još druga farmakološka sredstva koja uključuju traneksamičnu kiselinu, koncentrat fibrinogena i koncentrat protrombinskog kompleksa. Vaţno je takoĎer pratiti promjene u tjelesnoj temperaturi bolesnika i odgovarajuće ih korigirati. Danas postoje različiti protokoli za masivnu transfuziju kojima je cilj omogućiti bolju skrb za bolesnika s masivnim krvarenjem. Primjena krvi i krvnih pripravaka nosi sa sobom rizik za razvoj različitih transfuzijskih komplikacija.
Abstract (english)
There isn't an universal definition of massive bood loss. Common causes of massive bleeding include trauma and major elective surgical procedures. Significant blood loss leads to the development of hemorrhagic shock and coagulopathy. Hemorrhagic shock is associated with decreased tissue perfusion, ischemia, cell necrosis, and if not treated in time it will lead to multiorgan failure and death. Coagulopathy is multifactorial in origin and in combination with acidosis and hypothermia it forms a ''lethal triad''. In order to monitor and identify changes in the patient's coagulation system, the application of tests such as PT, aPTT, measuring the concentration of fibrinogen, rotational thromboelastometry and thromboelastography is important. In the treatment, it is important to identify the source of the bleeding and to establish control of the bleeding. At the beginning of the resuscitation therapy infusions of crystalloids and colloids should be applied. In the regulation of coagulopathy other pharmacological agents are used, including: tranexamic acid, fibrinogen concentrate and prothrombin complex concentrate. It is also important to monitor changes in body temperature of the patient and correct them appropriately. Today there are different massive transfusion protocols which aim to enable better care for patients with massive bleeding. The use of blood and blood products carries the risk of causing various transfusion complication.
